伽玛刀联合介入治疗原发性大肝癌15例疗效分析 被引量:2

摘要 目的探讨不能手术的原发性大肝癌或孤立性巨块型肝癌患者采用伽玛刀联合介入治疗的近期疗效。方法 15例不能手术的肝癌患者(肝肿瘤最大径8-16cm)先行伽玛刀治疗,45%-50%等剂量曲线,3.0-3.5Gy/次,1次/d,5次/周,35-42Gy/8-12次。放疗结束1月后采用肝动脉插管介入治疗2-3次(平均50d介入1次)。介入治疗药物为5-氟尿嘧啶、阿霉素(或表阿霉素)及顺铂。结果 2次介入治疗结束后4周复查腹部增强CT或MRI评价疗效,总有效率(CR+PR)为60.0%(9/15)。肝功能生化指标无显著变化,门静脉癌栓缩小或消失。治疗过程中无明显急性放射性损伤。结论对不能手术的原发性大肝癌或孤立性巨块型肝癌,伽玛刀联合介入治疗,能明显提高患者的局控率,达到了姑息性治疗的目的。 Objective To analyze the application of a new reagent in Bio-Rad variant II HbA1c analyzer.Methods Accuracy,linear analysis of diluted sample,whole blood and diluted samples,and stability of diluted samples of the new reagent,as well as its correlation with the old reagent were evaluated.Results The between-run and within-run accuracy of diluted sample was ≤1.4% and ≤0.93%,respectively.The accuracy of whole blood sample was higher than that of diluted sample.The linear theoretical value of diluted sample was well correlated with its measured value(r=0.998 5,P0.01).No statistical difference was found in diluted and whole blood samples(P0.05).However,a statistical difference was observed in diluted and whole blood samples of the old reagent(P0.01).The value of diluted sample was higher than that of whole blood sample.The diluted samples kept at room temperature for 10h were stable,with no difference with those kept at room temperature for 0h(P0.05).A rather good linear correlation was observed between the new and old reagents(r=0.994 2,P0.01 for whole blood;r=0.995 2,P0.01 for diluted blood).No statistical difference was found in whole blood sample of the new and old reagents(P0.05).However,a statistical difference was observed in diluted sample(P0.01).Conclusion The new reagent is better than the old reagent for Bio-Rad Variant II HbA1c assay and can meet clinical requirements better than the old reagent.
