住院病历用药合理性分析

Analysis of the Rationality of Drug Use in Inpatient Case Histories

目的:了解湖南省邵阳市中心医院住院病历的合理用药状况。方法:2009年9月—2010年4月,每个科室随机抽取10份出院病历(共2480份出院病历),进行统计分析。结果:2100份病历用药基本合理。380份病历中有680处不合理用药现象,主要包括用法不合理、诊治不符及选药不合理、预防用药使用时机不对、溶剂使用不合理、重复用药、配伍不当及相互作用。结论:住院病历存在着诸多的用药缺陷,需要管理部门和临床药师紧密配合,加大考核力度,促进合理用药。

OBJECTIVE：To investigate the rational drug use condition in the inpatient case histories of Hunan Shaoyang Municipal Central Hospital.METHODS：From Sept.2009 to Apr.2010,10 discharge case histories were sampled form each department（2 480 in total） for a statistical analysis.RESULTS：The drug use in 2 100 cases was basically rational.In the 380 irrational cases histories,the irrational drug use were noted in 680 points,which chiefly included irrational administration,nonconformity between diagnosis and treatment,irrational drug selection,improper preventive medication occasion,irrational use of solvents,repeated drug use,incompatibility of medicines and interactions.CONCLUSION：There were many defects in drug use in the inpatients case histories.To promote rational use of drugs,the administrative department and clinical pharmacists should cooperate well with each other and intensify evaluation.
