上唇腺泡细胞癌临床分析并文献复习

Acinic cell carcinoma of the upper lip： clinical analysis and literature review

目的 探讨小涎腺腺泡细胞癌的临床表现、病理特点、治疗方法和预后.方法 对1例腺泡细胞癌患者的临床资料进行总结并结合国内外已发表有关腺泡细胞癌临床和治疗特点的文献进行复习.结果 1例81岁男性患者主诉上唇肿物3年.患者3年前发现上唇黏膜长一肿物,约黄豆粒大小,无疼痛等自觉症状,未进行治疗,就诊时肿物渐增大1个月,影响上唇运动及美观就诊前1周,肿物曾自行破溃,流出血性分泌物前来就诊.入院后局麻下行上唇肿物切除术,术后病理为上唇腺泡细胞癌,局部侵犯表皮形成溃疡,未见明确脉管及神经侵犯,肿瘤未侵犯基底部横纹肌组织.结论 唇腺泡细胞癌临床极少见,术前应注意与基底细胞腺瘤和多形性腺瘤鉴别.

Objective To investigate the clinical and pathological characteristics of acinic cell carcinoma and to discuss its treatment and prognosis. Methods Acinic cell carcinoma （ACC） diagnosed in beijing chaoyang Hospital was retrospectively analyzed and the literature published was reviewed. Results ACC is a slow growing,low-grade malignancy of the salivary glands. Most cases occur in the parotid, but a few cases affecting the minor salivary glands have also been described, cases reported in the palate glands were more than in the labial glands. Generally think that ACC can occur at any age, but most cases occur in the elderly people and show a female predilection. The clinical feature of ACC were observed as following：indolent tumor, slow growing, long course of disease,like pleomorphic adenoma, lymp node metastasis was rare. Conclusion ACC in the labial glands is extremely rare. It＇s should be identified with basal cell adenoma and pleomorphic adenoma.
