全脑放疗联合同期推量调强技术治疗多发脑转移瘤7例 被引量:12

Whole Brain Radiotherapy Combined with Simultaneous Integrated Boost-IMRT in the Treatment for 7 Cases with Multiple Brain Metastases

摘要 [目的]探讨全脑调强放疗同期推量照射技术治疗多发性脑转移瘤的可行性。[方法]7例脑转移瘤患者,运用Corvus6.3逆向调强计划系统行调强同期推量照射技术(SIB-IMRT)治疗,全脑预防放疗剂量30Gy,转移瘤同期推量20Gy,均分10次,2周完成。计划应用Mattrix验证。[结果]所有病例靶区剂量分布满意,95%以上的计划靶区(PTV)体积接受处方剂量,99%以上的PTV体积接受93%的处方剂量,接受110%的处方剂量的体积不超过靶区体积的5%,重要器官的照射剂量限制均达到要求。剂量验证γ值为87.78~96.11,中位值92.25。7例患者均顺利完成治疗,未出现临床不可接受放疗不良反应。[结论]全脑调强放疗同期推量技术在临床、放射物理及安全性角度评价是可行的,对颅外转移病灶控制、KPS评分较好、原发病灶控制的脑转移瘤患者可推荐使用。
