预激综合征并心房颤动心电图特征分析

ECG Characteristics of Atrial Fibrillation in Patients with WPW Syndrome

目的分析探讨预激综合征合并心房颤动的心电图特征及鉴别诊断。方法分析12例预激综合征合并心房颤动心电图的f波,R-R间期的变化范围,心室率及QRS波形态的特点。结果 12例预激综合征中A型预激9例,B型预激3例,其中6例可见f波,R-R间期变化范围在0.20～1.0s(1.0s为起搏间期),心室率在60～300次/min,QRS波6例呈完全预激图形,2例呈完全典型预激,4例大部分呈完全预激图形,少部分呈典型预激图形或正常图形。结论预激综合征合并心房颤动的心电图是:较快的心室率甚至可达极速的心室率,R-R不齐。QRS波增宽起始呈delta波,QRS波可表现为完全预激图形,典型预激图形及正常QRS波的不同组合。

Objective To analyze and explore the Electrocardiographic features and differential diagnosis of atrial fibrillation in patient with WPW syndrome.Method To analyze the characteristics of f wave,RR interval variation range,ventricular rate and QRS wave morphology in patients with 12 patients with atrial fibrillation and WPW syndrome 9 cases of type A and 3 cases of type B in 12 patients with WPW syndrome,of which f wave were seen in 6 cases,RR interval changes in the range 0.20 ~ 1.0s（1.0s as the pacing interval）,ventricular rate of 60 ~ 300 times / min,in additional,QRS wave was fully graphical in 6 cases,and were entirely typical of pre-excitation in 2 cases,and most of the graphics were completely pre-excitation in 4 cases,a typical pre-shock graphics or normal graphics in a small number of patients.Results WPW syndrome with atrial fibrillation ECG： ventricular rate even faster up to speed the heart rate,RR missing.Widened QRS wave delta wave was starting,QRS wave can be expressed as full pre-excitation graphics,the typical pre-shock wave graphics and different combinations of the normal QRS.
