体外膜肺氧合对内毒素诱导大鼠急性肺损伤的早期干预效果 被引量:4

摘要 目的观察体外膜肺氧合(ECMO)对内毒素(LPS)诱导大鼠急性肺损伤(ALI)早期干预的肺保护作用。方法 LPS静脉注射复制ALI模型。30只健康雄性SD大鼠随机分为3组:对照组(C组,右颈外静脉注射生理盐水)、LPS刺激组(L组,右颈外静脉注射LPS)、ECMO干预组(LE组,右颈外静脉注射LPS,2 h后采用右颈外静脉—右股动脉转流2 h),每组10只。于静脉注射前5 min(T0)、注射后1 h(T1)、2 h(T2)、3 h(T3)、4 h(T4)记录大鼠的呼吸频率、动脉血氧分压(PaO2)。T4时观察大鼠两肺大体病变,在光学显微镜下观察肺组织的病理改变,测定肺组织湿/干重比。结果与T0时比较,T2、T3、T4时L组呼吸频率加快,T4时LE组呼吸频率减慢(P<0.01);与C组比较,L组T2、T3、T4时呼吸频率加快,LE组T2时呼吸频率加快(P<0.05)、T4时呼吸频率减慢(P<0.01);与L组比较,LE组T3、T4时呼吸频率减慢(P<0.01)。与T0时比较,T1时L组血氧分压升高(P<0.01),T3、T4时L组血氧分压下降(P<0.01),T1、T3、T4时LE组血氧分压升高(P<0.01);与C组比较,L组T1时血氧分压升高(P<0.01),T3、T4时血氧分压下降(P<0.01),LE组T1、T3、T4时血氧分压升高(P<0.01);与L组比较,LE组T3、T4时血氧分压升高(P<0.01)。与C组比较,L组的肺组织湿/干质量比值增加(P<0.01)。光镜下可见L组表现为严重的ALI的病理学变化,LE组表现为较轻度的ALI的病理学变化。结论 ECMO在LPS诱导大鼠ALI早期进行干预,有肺保护作用。
