摘要
目的:探讨永存镰状窦的MR表现,提高对永存镰状窦的认识和诊断率。方法:收集2005年1月至2010年10月9例临床及影像学确诊为永存镰状窦患者的临床及影像学资料,结合文献进行回顾性分析,研究永存镰状窦的MR表现。结果:9例永存镰状窦患者,常规MRI均显示大脑镰后部条状流空血管影自大脑大静脉向上引流至上矢状窦中、后三分之一处,其中1例合并顶枕部脑膜膨出、直窦缺如及小脑幕发育异常,1例仅合并直窦发育不良,3例合并动静脉血管畸形,1例合并低颅压综合征,余3例表现为孤立永存镰状窦。磁共振静脉成像(MRV)清晰显示大脑大静脉经开放的镰状窦向上引流入上矢状窦。三维时间飞越法磁共振血管造影(3D-TOF MRA)、增强扫描T1WI及磁敏感加权成像(SWI)静脉重建图像也可清晰显示永存镰状窦的形态及走行。结论:永存镰状窦MR表现具有特征性,常规MRI与MRV、MRA及SWI等多种扫描技术相结合可以更好地反映病变血管的形态、位置及其与周围结构的关系,有助于对永存镰状窦全面综合评价。
Objective:The objective of this study was to investigate the MR characteristics of persistent falcine sinus,in order to improve the understanding and diagnosis of persistent falcine sinus.Methods:Clinical and MRI data were collected from 9 patients with persistent falcine sinus confirmed by MRI from January 2005 to October 2010.Results:MR cerebral conventional examination was performed in all 9 cases,showing an enlargement vascular flow void arising from the great cerebral vein and draining into the superior sagittal sinus in its middle and posterior third.Persistent falcine sinus was associated with parietooccipital meningocele,absence of straight sinuses,and dysplastic tentorium in 1 patient,only with straight sinuses agenesis in 1 patient,with arteriovenous malformations in 3 patients,with intracranial hypotension syndrome in 1 patient and the remaining 3 were isolate persistent falcine sinus.Magnetic resonance venous imaging(MRV) can clearly show the great cerebral vein draining into the superior sagittal sinus through the opening falcine sinus.3D-Time of Flight Magnetic Resonance Angiography(3D-TOF MRA),enhanced T1WI and Susceptibility-weighted imaging(SWI) also clearly show the appearance and pathway of the persistent falcine sinus.Conclusion:MR performance of persistent falcine sinus is characteristic.Conventional MRI combined with MRV,MRA and SWI can show more clearly the appearance,position of the persistent falcine sinus and its relationship with surrounding structures,contributing a lot to its comprehensive evaluation.
