肾上腺皮质激素对血小板输注效果的影响

The effect of adrenocorticosteroid on platelet transfusion

目的 研究肾上腺皮质激素对血小板输注效果的影响。方法 采用随机对照试验，实验组１１ 例血液病患儿在输注血小板前半小时输注氢化考的松（７－５ ｍｇ／ｋｇ），对照组８ 例血液病患儿不输注氢化考的松，两组均在输注血小板后１ 、１８、４２ 及６６ 小时分别进行外周血血小板计数，计算纠正计数指数（ＣＣＩ） 及回升率，并观察临床疗效，对比两组的输注效果。结果 试验组ＣＣＩ、回升率均明显高于对照组（ Ｐ＜０－０５），其止血无效率、输血反应发生率及输注无效率均低于对照组。结论 肾上腺皮质激素的应用可提高血小板输注的疗效，增加输注后的ＣＣＩ及回升率，减少输注无效性的发生。

Objective To explore the effect of adrenocorticosteroid on platelet transfusion Methods In the Random Controlled Test study, 11 hematopathy children injected with hydrocortisone (7 5 mg/kg) were chosen as experimental group, and other 8 hematopathy children without the injection of hydrocortisone as control group. All children were then transfused with platelet, and their platelet counts as well as corrected count index (CCI) and recovery rate were calculated at 1, 18, 42 and 66 hours after platelet transfusion Results CCI and recovery rate in the experimental group increased significantly than those in the control group ( P< 0 05) The incidence of transfusion reactions and transfusion refractoriness in the experimental group were lower than those in the control group Conclusion These results suggested that adrenocorticosteroid could improve the effect of platelet transfusion, increase CCI and recovery rate after transfusion, and decrease the incidence of transfusion refractoriness
