肿瘤相关糖蛋白-72在结肠癌及癌前病变组织中的表达

Expression of tumor associated glycoprotein-72 in colorectal carcinoma and precancerous lesions

目的观察肿瘤相关糖蛋白-72(TAG-72)抗原在结肠癌及癌前病变组织中的表达,并探讨其临床意义。方法应用免疫组化EnVision二步法检测TAG-72抗原在221例结肠癌及癌前病变组织中的表达,其中癌旁正常结肠黏膜32例、结肠增生性息肉24例、结肠低级别上皮内瘤变75例、结肠高级别上皮内瘤变和结肠管状腺癌各45例。结果 TAG-72抗原在正常结肠黏膜、增生性息肉、低级别上皮内瘤变、高级别上皮内瘤变及腺癌组的表达阳性率分别为3.12%、29.17%、53.33%、88.89%、100%。正常组与息肉组间表达率差异不显著(P>0.05);低级别、高级别上皮内瘤变组、腺癌组抗原表达率显著高于前两组(P<0.05);其中高级别上皮内瘤变组与腺癌组的抗原表达明显增高,但两组间表达差异不显著(P>0.05)。结论 TAG-72在结肠高级别上皮内瘤变及结肠癌中表达增高。由于TAG-72在结肠组织的恶性转化中起着重要作用,故其阳性表达及表达强度对诊断结肠腺癌和监测腺瘤癌变有重要意义。

Objective To investigate the tumor associated glycoprotein-72（TAG-72） expression in the colorectal carcinoma and precancerous lesions and its clinical significance.Methods Immunohistochemistry（EnVision） was utilized to detect the expression of TAG-72 in 32 cases of normal colon epithelium,24 cases of hyperplastic polyposis of the colon,75 cases of low-grade intraepithelial neoplasia,45 cases of high-grade intraepithelial neoplasia,and 45 cases of colon carcinoma.Results TAG-72 expression in the above 5 groups were 3.12%,29.17%,53.33%,88.89% and 100%,respectively.There was no relation in TAG-72 expression between normal colon epithelium and hyperplastic polyposis（P〉0.05）.TAG-72 expression in low-grade,high-grade intraepithelial neoplasia and colonic carcinoma were higher than that in other groups（P〈0.05）;that in high-grade intraepithelial neoplasia and carcinoma were much higher than other groups（P〈0.01）,but there was no significant difference between high-grde intraepithelial neoplasia and carcinoma groups（P〉0.05）.Conclusion TAG-72 expression is up-regulated in high-grade intraepithelial neoplasia and carcinoma.TAG-72 plays an important role in malignant transformation of the colon.The positivity and intensity of TAG-72 expression have important implications to the diagnosis of adenocarcinoma and the monitor of malignant transformation.
