摘要
目的:探讨2型糖尿病合并无症状性腔隙性脑梗死(SLCI)的危险因素。方法:选择2型糖尿病患者165例,按照是否同时存在SLCI,分为SLCI组(55例)和对照组(110例)。收集患者的生化检测指标,尿微量白蛋白(MAU)、腰臀比(WHR)、体重指标(BMI)、糖尿病病程以及是否存在吸烟、饮酒、冠心病病史等危险因素。结果:①与对照组比较,SLCI组患者的三酰甘油[TG:1.7(1.4)mmol/L、1.5(1.1)mmol/L]、尿微量白蛋白[27.5(11.0)mg/24 h、7.0(5.5)mg/24 h]、红细胞体积分布宽度[12.5+0.7]、(12.2+0.6]、腰臀比[(0.94+0.08)、(0.88+0.09)]、体重指数[(27+13)kg/m2、(25+3)kg/m2]均高于对照组,差异有统计学意义,P〈0.05;②Logistic多元回归分析显示,尿微量白蛋白(OR:1.482;95%,CI:1.067~2.060)红细胞体积分布宽度(OR:12.599;95%,CI:1.164~136.358)是本研究中2型糖尿病合并SLCI的独立危险因素。结论:2型糖尿病合并SLCI患者的尿微量白蛋白、红细胞体积分布宽度明显增加,是2型糖尿病合并SLCI的独立危险困素。
Objective To investigate the risk factors for essential type 2 diabetes mellitus Silent Lacunar Infarction(SLI).Methods A total of 165 patients with type 2 diabetes mellitus were recruited in the study.They were divided into SLI group and control group according to whether they had SLI.The biochemical detection index microalbuminuria(MAU),waist to hip ratio(WHR),body mass index(BMI),duration of diabetes mellitus,as well as whether they had the following risk factors,such as history of smoking,drinking,and coronary heart disease,were collected.Results ①Triglyceride(M:1.7 mmol/L vs,1.5mmol/L,microalbuminuria(27.5 mg/24h vs,7.0 mg/24h),red WHR(0.94±0.08 vs,0.88±0.09),and BMI(27±13kg/m2 vs,25±3 kg/m2) in the SLI group were significantly higher than those in the control group.There were significant differences between the two groups(P0.05).②A Multiple Logistic regression analysis shown that MAU,red cell volume distribution width(RDW),were the independent risk factors for diabetes mellitus complicating SLI in this study.Conclusion The significantly increased MAU and RDW in patients with diabetes mellitus complicating SLI are the independent risk factors for diabetes mellitus complicating SLI.Using MAU and RDW determination to evaluate the risk for diabetes mellitus patients complicating lacunar infarction have important significance.
