早期肠内肠外营养在胃十二指肠患者术后的临床支持效果 被引量:2

摘要 目的:探讨胃十二指肠患者手术后早期肠内肠外营养(EEN-PN)的临床支持效果。方法:将46例胃十二指肠手术治疗的患者随机分为EEN-PN组和全肠外营养(TPN)组,EEN-PN组于术中将肠内营养管置入空肠内,术后第1天予以肠内营养(EN),患者能较正常地口服营养液或进食半流饮食后停止;TPN组于术后进行常规输液治疗,待肠蠕动完全恢复,肛门排气后开始进食。分别于术前1d和术后7d测量患者体质量,检测血清清蛋白、血红蛋白,记录肛门排气时间、住院时间和住院费用,观察术后有无吻合口漏、切口感染、腹胀、腹痛等并发症。结果:肛门排气时间EEN-PN组为(48±6)h,TPN组为(72±8)h,差异有统计学意义(t=2.358,P<0.05);EEN-PN组住院时间、住院费用均显著少于TPN组(P<0.01);EEN-PN组术后胃肠功能恢复、体质量维持等方面明显优于TPN组。结论:胃十二指肠手术后早期肠内肠外联合营养治疗安全可靠、简便易行、符合生理、肠功能恢复快、并发症少、费用低廉。
