腹腔镜在大隐静脉曲张手术中的应用分析

Application analysis of laparoscopic surgery for great saphenous vein varicosity

目的探讨腹腔镜下交通支静脉离断术治疗下肢大隐静脉曲张的疗效。方法以该院血管外科27例41条下肢大隐静脉曲张患者为治疗组,以同期收治的35例42条下肢大隐静脉曲张患者为对照组。对照组给予大隐静脉高位结扎及曲张静脉剥脱术,治疗组在传统下肢静脉曲张高位结扎抽剥的基础上给予腹腔镜筋膜下交通静脉离断手术。比较两组疗效。结果治疗组平均手术时间短于对照组,差异具有统计学意义(P<0.05);出院后随访1～24个月,治疗组复发1例,复发率4.88%,对照组复发8例,复发率19.05%,治疗组复发率低于对照组,差异具有统计学意义(P<0.05)。结论腹腔镜下交通支静脉离断术治疗下肢大隐静脉曲张疗效满意,能避免和减少术后复发,值得临床推广。

Objective To evaluate the efficacy of subfascial endoscopic perforating surgery for great saphenous vein varicosity of lower extremity.Methods 27 cases of great saphenous vein varicosity of lower extremites including 41 limbs in the department of vascular surgery in our hospital were as the therapeutic group.35 cases of great saphenous vein varicosity of lower extremites including 42 limbs were as the control group.The control group was given the high ligation of great saphenous vein and stripping of varicose vein.The therapeutic group was given subfascial endoscopic perforating surgery in the basis of traditional high ligation stripping of varicose veins.To compare the efficacies between the 2 groups.Results The mean operative time of the therapeutic group was shorter than that of the control group with significant difference（P0.05）.All cases were followed up for 1~24 months.1 case recurred in the therapeutic group with the recurrence rate of 4.88%,while 8 cases in the control group with the recurrence rate of 19.05%.The recurrence rate of the therapeutic group was lower than that of the control group,showing significant difference（P0.05）.Conclusion Subfascial endoscopic perforating surgery for treating great saphenous vein varicosity of lower extremites has satisfactory effect,which can avoid and reduce recurrence.It was worthy of promotion in clinic.
