两种胰岛素强化治疗方案对2型糖尿病的疗效观察 被引量:29

摘要 目的观察甘精胰岛素联合门冬胰岛素(Ⅰ组)与诺和灵R联合诺和灵N(Ⅱ组)两种胰岛素强化治疗方案对2型糖尿病糖代谢的影响。方法两组患者各30例,Ⅰ组三餐前即刻皮下注射门冬胰岛素,睡前皮下注射甘精胰岛素,Ⅱ组三餐前30min皮下注射诺和灵R,睡前皮下注射诺和灵N,根据血糖调整胰岛素用量,使血糖达标,观察治疗后空腹血糖、餐后2h血糖、血糖达标时间、每日胰岛素用量、低血糖发生率,并应用动态血糖监测系统(CGMS)评价血糖波动,计算日内平均血糖波动幅度(MAGE)及日间血糖平均绝对差(MODD)。结果两组治疗方案均能良好控制空腹血糖、餐后2h血糖及平均血糖(MBG),但Ⅰ组的血糖达标时间、每日胰岛素用量、低血糖发生率、MAGE及MODD明显小于后者(P<0.05)。结论甘精胰岛素联合门冬胰岛素强化治疗能更好地控制2型糖尿病患者的糖代谢状况。
