无痛胃镜检查对老年患者血压、心率和血氧饱和度的影响

Effects of analgesic gastroscopy on blood pressure,heart rate and blood oxygen saturation in aged patients

目的:观察无痛胃镜检查对老年患者血压、心率和血氧饱和度的影响,了解老年患者行无痛胃镜检查的安全性。方法:无痛胃镜检查是在胃镜检查前先予以芬太尼及丙泊酚静脉麻醉。以多功能监护仪监测并记录无痛胃镜检查前、检查中、苏醒时和休息10min患者的收缩压、舒张压、心率和血氧饱和度,并记录不良反应(呛咳、呃逆、呼吸抑制等)。结果:与检查前比较,检查中收缩压[(139.1±4.5)mmHg∶(123.6±5.0)mmHg]、舒张压[(82.6±5.7)mmHg∶(72.8±4.1)mmHg]、心率[(77.3±3.4)次/min∶(69.2±4.4)次/min]和血氧饱和度[(98.2±0.9)%∶(95.6±0.5)%]均明显下降(P均<0.05),苏醒时收缩压[(128.4±6.3)mmHg]和舒张压[(77.3±7.3)mmHg]仍明显低于检查前(P<0.05),但休息10min各指标恢复,无严重合并症发生。结论:无痛胃镜检查适用于老年患者,但检查过程中应密切观察生命体征的变化。

Objective：To observe effects of analgesic gastroscopy on blood pressure,heart rate and blood oxygen saturation in aged patients and explore its safety.Methods：Before gastroscopy was performed,intravenous fentanyl and propofol were received for patients.Multifunction monitor was used to measure systolic blood pressure（SBP）,diastolic blood pressure（DBP）,heart rate（HR） and blood oxygen saturation（SpO2） of patients before and during analgesic gastroscopy,recovery and 10min after rest.Record the parameters at each timing point and adverse reaction（cough,hiccup,respiratory depression and so on）.Results：Compared with before gastroscopy,the SBP [（139.1±4.5） mmHg vs.（123.6±5.0） mmHg],DBP [（82.6±5.7） mmHg vs.（72.8±4.1） mmHg],HR [（77.3±3.4） times/min vs.（69.2±4.4） times/min] and SpO2 [（98.2±0.9）% vs.（95.6±0.5）%] significantly decreased in course of gastroscopy,P0.05 all.When the patients waked up,their SBP [（128.4±6.3） mmHg] and DBP [（77.3±7.3） mmHg] still significantly lower than before gastroscopy（P0.05）,but all parameters recovered after 10min rest and no severe complication occurred.Conclusion：Analgesic gastroscopy is feasible and safe for aged patients but vital signs should be observed closely during practice.
