双钢板内固定治疗粉碎性肱骨远端骨折34例

Double-plate Fixation to Treat Comminuted Distal Humeral Fracture(34 Cases)

目的评价双钢板内固定治疗粉碎性肱骨远端骨折的效果。方法对我院34例粉碎性肱骨远端骨折采用AO重建双钢板固定技术行开放复位内固定,并早期进行功能锻炼。结果本组术后随访1～3年,5例失访。获随访的29例桡骨小头、尺骨冠状突、鹰嘴及肱骨髁上骨折均愈合,其中6例严重粉碎骨折1.5～2.0年延迟愈合,2例内髁骨折未愈合。无感染及肘内翻出现。根据Cassebaun评分系统对本组进行术后疗效评定:优10例,良12例,可3例,差4例。结论双钢板内固定治疗粉碎性肱骨远端骨折方法可靠,术后并发症少,肘关节功能恢复满意。

Objective To evaluate the clinical outcome of double-plate fixation in treating comminuted distal humeral fracture.Methods 34 patients in our hospital with comminuted distal humerus fracture were treated by AO reconstructive double-plate technique with open reduction and internal fixation,followed by early functional exercises.Results The patients were followed up for 1 to 3 years,and follow-up was lost in 5 patients.Of the 29 followed up patients,fractures of radial head,ulna coronoid,olecranon and humeral healed up;6 patients with severe comminuted fracture had delayed union in 1.5 to 2.0 years and 2 patients with medial condylar fracture had not healed at the last follow up.No infection or elbow varus developed.According to Cassebaun scoring system,the postoperative therapeutic evaluation of this group of patients were： excellent in 10 cases,good in 12,satisfactory in 3 and poor in 4.Conclusion Double-plate fixation of comminuted distal humeral fracture method is a reliable method with fewer postoperative complications and satisfactory functional recovery of elbow.
