摘要
目的:对临床各类标本中检出的阴沟肠杆菌的药物敏感性进行分析,为临床抗感染治疗制定合理的用药方案提供依据。方法:通过Vitek-AMS全自动微生物分析仪对临床各类标本中分离的58株阴沟肠杆菌进行检测,分析其对各种抗生素的敏感与耐药情况。结果:阴沟肠杆菌对亚胺培南的敏感率为100%;其次是氟喹诺酮类的左氧氟沙星、环丙沙星,均为63.8%;最低的是青霉素类的氨苄西林和头孢一代的头孢唑啉,分别为0%和3.4%。结论:阴沟肠杆菌对多种抗生素均有不同程度的耐药,显示其多重耐药机制较为复杂;碳青霉烯类(如亚安培南)对本菌具有极高的抗菌活性,可作为治疗本菌较严重感染的首选药物;而第三代氟喹诺酮类、四代头孢菌素类和加抑酶剂的青霉素类等药物对本菌仍保持一定的抗菌活性;头孢西丁与氨曲南也可作为治疗本菌较好的药物或联合用药较佳的选择者。
Objective:To study the antimicrobial activity of antibiotics to Enterobacter cloacae and to analyse its drug resistance,in order to better guide rational remedy in clinical infection.Methods:The medical sensitivity and antibiotics resistance in 58 strains of Enterobacter cloacae isolated from the various clinical samples were determined by Vitek-AMS.Results:The highest medical sensitivity rate of Enterobacter cloacae was imipenem(100%).The second was ciprofloxacin and levofloxacin,both were 64%.The lowest was ampicillin in penicillins and cefazolin,which was 0% and 3.4% respectively.Conclusion:Enterobacter cloacae shows the different levels of drug resistance to 13 antibiotics and its complex mechanism of multiple drug resistance.Imipenem possesses the high level of antimicrobial activity,which can be used for the first choice to cure more serious infection.The third generation fluoroquinolones(such as ciprofloxacin and levofloxacin),the fourth generation cephalosporins(such as cefepime) and ntienzyme penicillins(such as piperacillin/tazobactam) still maintain some antimicrobial activity to Enterobacter cloacae.Cefoxitin and aztreonan are also a good choose.
