目的研究冠状动脉疾病(CHD)超敏C反应蛋白(hs-CRP)的1d内变化及与CHD严重程度相关性,并研究不同时段其cutoff的灵敏度和特异性。方法对124个病人分成三组:正常人、轻度CHD、重度CHD,测量在1d中6时,12时,18时和24时的hs-CRP,并进行分析。结果即时hs-CRP显著性改变在6时浓度最高,12时最低(P<0.01)。所有重度CHD的hs-CRP的暂时值和绝对值都高(P<0.01)。不同时段cutoff值对重度CHD的预测具有相同的特异性和灵敏度。不同的cutoff与CHD的严重程度相关。结论无论有无CHD,其血清hs-CRP在24h是动态改变的,6时浓度最高,不同时段cutoff值对重度CHD的预测具有相同的特异性和灵敏度,并与严重程度相关。
Objective To investigated the variation of hs-CRP levels in 24h and their association with the severity of CHD in patients with coronary heart disease (CHD), and study the sensitivity and specificity of the cutoff value in different time. Methods We divided 124 patients into three groups:the normal,mild CHD,severe CHD,and measured 5h,12h,18h and 24h hs-CRP levels,in 1 day then analyzesed. Results Temporal hs-CRP levels varied significantly-the highest mean concentrations were found 6h,and the lowest concentrations 12h (P〈0.01). All temporal hs-CRP measurements and the absolute increase in hs-CRP levels were significantly higher in patients with severe CHD (both P〈0.01 ). It showed that cutoff values of hs-CRP taken at different times predicted severe CHD with similar sensitivity and specificity. Different cutoff values for temporal hs-CRP levels correlated with the severity of CHD. Conclusion Serum levels of hs-CRP varied over 24 hours,whether patients had CHD or normal coronary arteries, cutoff values of hs-CRP taken at different times predicted severe CHD with similar sensitivity and specificity. Different cutoff values for temporal hs-CRP levels correlated with the severity of CHD.
