18F-FDG PET/CT Compare with CT on Evaluating Chemotherapy Reaction in Treatment Early Cisplatin in Solid Tumer in Rabbits

摘要 目的评价PET/CT能否用于早期检测化疗的敏感性,探讨化疗后行PET/CT进行疗效评价的理想时间。方法 28只右后腿接种VX2实体瘤的兔模型随机分成治疗组和对照组,每组7只兔子,治疗组分3组在静脉给予顺铂前进行第1次PET/CT显像,36h后行第2次PET/CT显像,治疗组分别在第2次显像前4h、前24h与前36h静脉给予6mg/kg顺铂;对照组不给予化疗药物,显像时间与实验组相同。取葡萄糖摄取最大值(SUVmax)进行分析,利用CT测量肿瘤的体积。结果化疗4h后显像组SUVmax减低率为(-11.11±13.13)%,化疗24h后显像组SUVmax减低率为(-37.33±12.58)%,化疗48h后显像组SUVmax减低率为(-34.21±12.93)%。3个实验组与对照组间SUVmax变化率均有显著性差异(P<0.05)。3个实验组与对照组间体积变化率均无显著性差异(P>0.05)。结论在兔VX2移植瘤动物模型建立的基础上,给予顺铂化疗后不同时间点进行PET/CT显像,发现化疗后很早期(4h)瘤体SUVmax即有明显变化,而此时利用CT测量的肿瘤体积变化不明显。表明PET/CT优于传统影像方法,能够在体、早期、灵敏检出肿瘤对化疗药物的敏感性。 Objective To explore the feasibility of using ISF-FDG PET/CT as an in vivo individual chemosensitivity testing method. Methods Twenty-eight VX2 rabbits were randomized into treatment group and control group.The first 18F-FDG PET/CT was performed the day before intravenous administration of cisplatin (6mg/kg) and the second was 36 hours later. In the experimental groups, cisplatin administration was 4 hours, 24 hours and 36 hours before the second PET/CT imaging.The control group, without cisplatin administration, received the 18F-FDG PET/CT imaging at the same time points. Maximum standardized uptake value(SUVmax) was analyzed. Tumor volume was defined in CT images. Results The results showed that there is a significant difference (P〈0.05) in SU- Vmax on hour 4 between it and the control group, with SUVmax decrease rate of (-11.11 ± 13.13)%. On hour 24, a significant difference in SUVmax was observed between it and the control grou, (P〈0.05)with SUV decrease rate of (-37.33 ± 12.58)%. The results also showed a significant difference(P〈0.05) in SUVmax on hour36 between the group and the control group, with SUVmax decrease rate of (-34.21 ± 12.93)%. No significant difference in tumor volume was found between the three experimental groups and the control group (P〉0.05). Conclusion The study showed that 18F-FDG PET/CT can be used as an in vivo chemosensitivity testing method. According to the decrease rate when cisplatin adminstration, PET/CT can early in vivo sensitively differentiate the sensitive and insensitive tumors.
