摘要
目的回顾分析21例脑静脉系统血栓患者的临床表现与神经影像学特征，探讨早期诊断的规律。方法分析收治21例脑静脉系统血栓患者的临床与影像学资料，其中单纯脑深静脉系血栓1例，单纯皮层静脉血栓3例，静脉窦血栓17例。结果男性稍多见，年龄18～65岁，大多数呈急性或亚急性病程，先后或同时出现颅高压、癫痫及局灶神经功能障碍。急性期CT：静脉窦内高密度血栓影5例，随后出现静脉出血性梗死13例。急性期MRI T1WI增强扫描可见血栓的静脉窦充盈缺损4例，亚急性期平扫可见静脉窦内高信号血栓影11例。20例DSA脑血管造影可见静脉窦及／或相应的脑静脉闭塞或缺如，侧支静脉叫流增多，1例单纯皮层静脉血栓经开颅探查确诊。结论熟悉脑静脉系统血栓的临床表现以及静脉性脑梗死或出血性梗死的CT特征，及时行MRI及（或）DSA是早期诊断的关键。
Objective To investigate the early diagnosis of cerebral venous thrombosis（CVT）. Methods The clinical and image feature material in 21 patients of CVT were analyzed retrospectively. Results The onset may be acute or suhacute,the first and most common symptoms include intracranial hypertension,seizures and focal deficits,CT can find hyperdensity in venou sinus or haemorrhagic infact,MRI showed cerebral venous sinus occlusions,DSA showed lack of typical high flow siginal,formation of collateral circulation. Conclusion Familiar with the clinic and CT feature of CVT,the early diagnosis can be furthur estabilished by MRI and DSA examination.
