2009年临床分离鲍曼不动杆菌的标本分布及耐药性分析

Distribution and antibiotic resistance of Acinetobacter baumannii in Hospital

目的了解我院2009年鲍曼不动杆菌的耐药情况,以给临床合理用药提供参考。方法收集临床分离的鲍不动杆菌培养鉴定,采用纸片扩散法(κ-B法)或微量稀释法(MIC)测定20种抗菌药物的敏感情况,用WHONET 5软件进行分析。结果鲍不动杆菌对所有抗菌素均产生一定的耐药性。耐药率较高者依次递减分别为氨曲南(80.36%)、头孢哌酮(74.11%)、四环素(64.29%)、头孢噻肟(62.05%)、替卡西林/棒酸(62.05%)、复方新诺明(61.61%)、头孢吡肟(61.16%)、哌拉西林(60.71%)、头孢他啶(60.27%)、环丙沙星(59.38%)、哌拉西林/他唑巴坦(58.93%)、左旋氧氟沙星(58.04%)、亚胺培南(57.14%)、美洛培南(56.25%)、庆大霉素(54.91%)、米诺环素(47.32%);耐药率较低者依次递减分别为头孢哌酮/舒巴坦(30.80%)、丁胺卡那(27.68%)、妥布霉素(26.79%)、多粘菌素B(0.45%)。结论鲍曼不动杆菌对多种抗菌药物具有高度耐药性,应根据药敏结果合理选用抗菌药物。

OBJECTIVE To understand the distribution and antibiotic resistance of Acinetobacter baumannii（ABA）isolated from our hospital and offer suggestions for clinic.METHODS The antibiotic susceptibility to 20 kinds of antibiotics were tested by κ-B method and analyzed by Whonet software.RESULTS The higher drug-resistance of ABA strains to aztreonam,cefoperazone,ambramycin,cefotaxime,ticarcillin/clavulanic acid,cotrimoxazole,cefepime,piperacillin,ceftazidime,ciprofloxacin,piperacillin/tazobactam,levofloxacin,imipenem,meropenem,gentamicin,minocycline were 80.36%,74.11%,64.29%,62.05%,62.05%,61.61%,61.16%,60.71%,60.27%,59.38%,58.93%,58.04%,57.14%,56.25%,54.91%,47.32% respectively.The lower drug-resistance of ABA strains to cefoperazone/sulbactam,amikacin,tobramicin,Polymyxin B were 30.80%,27.68%,26.79%,0.45% respectively.CONCLUSIONS ABA isolated from our hospital showes high resistance to several kinds of antibiotics.The drugs should be choosed reasonably according to their antibiotic suscepitibity results.
