85例老年结核性胸膜炎的临床分析

The clinical analysis of 85 elderly cases with tuberculous pleurisy

目的 总结老年结核性胸膜炎的特点，减少误诊，并探讨治疗方法。方法 对１９９５ 年１ 月～１９９８ 年６ 月我院住院８５ 例老年结核性胸膜炎采用回顾性分析。结果 症状不典型（ 无胸疼６９４ ％ ，无胸闷６５８ ％ ，无发热５６５ ％ ） 较多见；并发症多；实验室检查无特异；易延误诊断； Ｃ Ｔ 检查及胸膜活检有助诊断。结论 掌握老年结核性胸膜炎临床特点，早期诊断，对疑难病例，应适时进行 Ｃ Ｔ 检查及胸膜活检，以减少误诊，合理治疗，可取得较好疗效。

Objective The characteristics of tuberculous pleurisy in elderly patients were concluded in order to inducemisdiagnosis and evaluate the treatment methods. Methods 85 cases oftuberculous pleurisyin elderly patientsfrom Jan .1995to June 1998 in our hospital were reviewed and analyzed . Results Atypicalsymptoms :no chest pain (69 4 % ) ,no chest de press (65 8 % ) ,no fever (56 5 % ) ; more associated diseases ; Chest C Texamination and pleural biopsy could assistin diagno sis . Conclusion Master the characteristics oftuberculous pleurisy in elderly patients,emphasis must be put on early diagnosisand C Texamination or pleural biopsy wassuggested forthose patients with different diagnosis,reducing misdiagnosis,and reg ular therapy receiving good results of treatment .
