间接门静脉数字减影造影对肝癌血供的研究

The studies on the blood supply of liver carcinoma in indirect portal vein digital subtraction angiography

目的研究肝癌有无门脉供血。材料与方法１１６例经临床及影像学资料证实的肝癌，采用经腹腔动脉或肠系膜上动脉行间接门静脉ＤＳＡ方法，观察肝癌有无门脉供血。结果１１６例肝癌中１０３例仅有肝动脉而无门静脉供血，只有１３例有肝动脉和门静脉双重供血（占１１．２％）。门脉供血表现为环状或少量染色。９７例原发性肝癌和１９例转移性肝癌有门脉供血者分别有１０例（占１０．３％）和３例（占１５．８％），两者无显著性差异（Ｐ＞０．０５）。结论间接门静脉ＤＳＡ以显示肝癌有无门脉供血，不受肝动脉血供的影响，符合血液动力学，其方法可靠。无论哪一类型肝癌均以肝动脉供血为主，门静脉仅参与少数肿瘤的部分血供。

Objective To study the portal vein blood supply of liver carcinoma in indirect portal vein DSA. Materials and Methods 116 cases with liver carcinoma, proved by clinical and image, were proceed indirect portal vein DSA through celiac artery or superior mesenteric artery. The portal vein blood supply of the tumor in all cases was observed. Results Among 116 cases with liver carcinoma, 103 cases have only heptatic artery blood supply, only 13 cases (11.2%) have double blood supply of heptaic artery and portal vein, the portal vein blood supply of the tumors showed circular or small amount staining. Double blood supplies in 97 cases with primary liver carcinoma and 19 cases with metastatic liver carcinoma were seen in 10 cases (10.3%) and 3 cases (15.8%). There is no significant difference between them (P>0.05). Conclusion Without influence of hepatic artery blood supply and abnormal hemodynamics, it is a reliable method to study the portal vein blood supply of liver carcinoma in indirect portal vein DAS. The blood supply of the tumor in all type of liver carcinoma is supplied by hepatic artery, only a little part of the tumor in a few cases is supplied by portal vein.
