摘要 由于胃癌预后的分子特征的作用还不清楚,本文旨在探讨hMLH1基因启动子CpG岛甲基化和微卫星不稳定(MSI)在胃癌预后的价值。我们对近年来关于hMLH1基因启动子CpG岛甲基化和微卫星不稳定(MSI)与胃癌关系的文献进行了综合分析。结果显示,MSI胃癌有着特殊临床病理学特征:老年女性,胃窦部位,肠型,较少浆膜侵犯,淋巴侵犯罕见,hMLH1基因启动子CpG岛甲基化,有较好的预后。因此可以得出结论,hMLH1基因启动子CpG岛甲基化和微卫星不稳定(MSI)在胃癌中有显著相关性,在胃癌早期发生、发展过程中起重要作用,对胃癌的鉴别诊断、预后评价、特殊临床干预具有重要的意义,可能成为胃癌早期诊断、评价预后、指导化疗的分子标志物。 The influence of molecular characteristics in prognosis of gastric cancer remains unclear. The aim of this study was to evaluate the prognostic value of the methylation of the hMLH1 promoter CpG island and mierosatellite instability (MSI) in gastric cancer. The literatures in recent years about the concept of them, their biological significance and the rela- tionship between microsatellite instability, the CpG island methylation and gastric cancer were reviewed. The effects of them on gastric cancer were also analyzed. The results shows that MSI gastric cancer have distinct clinicopathologic fea- tures such as eldly, female, antral tumor location, intestinal histotype, less infiltration of the serosa, less lymph node in- volvement, methylation of the hMLH1 promoter CpG island and are associated with a better prognosis. So we can draw the conclusion that there is a significant association between methylation of the hMLH1 promoter CpG island and MSI status in gastric cancer. They play an important role in the development and progression of gastric cancer. The status of them may be used as useful molecule markers for early diagnosis, assessing metastasis and evaluating prognosis, and could possibly direct chemotherapy for gastric cancer in the future.
