颅内动脉瘤103例分析

Clinical analysis of 103 cases' intracranial aneurysms

目的探讨颅内动脉瘤的临床表现及诊治经验。方法 回顾性分析2007年10月至2010年3月我院103例颅内动脉瘤患者发病、临床表现、影像资料。结果 103例患者中共查出126个颅内动脉瘤。其中单发84例,多发性颅内动脉瘤19例(15例有2个动脉瘤,4例有3个动脉瘤)。1例合并动静脉畸形,1例合并颈内动脉颅外段夹层动脉瘤,1例合并烟雾病。未破裂的颅内动脉瘤15例,占14.6%(15/103)。后交通动脉瘤最多见,占32.5%(41/126)。动脉瘤直径1.5～35mm不等,其中2.0～4.9mm者占61.9%(78/126)。结论 采用适宜的影像学检查及时诊断颅内动脉瘤并及时血管内治疗,可明显降低致死率和致残率。

Objective To investigate clinical manifestation,diagnosis,treatment of intracranial aneurysm and summarize the experience.Methods Retrospective analysis of 103 cases＇ intracranial aneurysms admitted in our hospital from Oct-2007 to Mar-2010 were carried out to study onset,clinical manifestation and image data.Results 103 patients were identified 126 intracranial aneurysms,in which 84 patients were single and 19 were multiple.15 patients had 2 aneurysms each and 4 patients had 3 aneurysms each.One patient combines arteriovenous malformation,one combines dissecting aneurysm of extracranial carotid artery and one combines Moyamoya disease.There were 15 cases of unruptured intracranial aneurysms,accounting for 14.6%（15/103）.Posterior communicating artery aneurysms were the most common,accounting for 32.5%（41/126）.Diameter of aneurysm was mostly found in 2.0-4.9 mm,accounting for 61.9%（78/126）,which the minimum is 1.5mm and the maximum is 35mm.Conclusions To apply appropriate and timely diagnostic imaging of intracranial aneurysm and endovascular treatment in time can significantly reduce mortality and morbidity
