抗环瓜氨酸肽抗体(Anti-CCP)与类风湿因子(RF)在类风湿性关节炎诊断中的探讨

摘要 目的探讨抗环瓜氨酸肽抗体与类风湿因子在类风湿性关节炎诊断上的应用。方法选择类风湿性关节炎患者74例,非类风湿性自身免疫性疾病患者55例,分别用酶联免疫吸附试验测抗环瓜氨酸肽抗体,用乳胶增强免疫比浊法测类风湿因子,分析各组抗环瓜氨酸肽抗体与类风湿因子有无相关性。结果类风湿因子对类风湿性关节炎敏感性为79.7%,特异性为52.7%。抗环瓜氨酸肽抗体对类风湿性关节炎敏感性为51.4%,特异性为96.4%。类风湿因子与抗环瓜氨酸肽抗体联合检测的敏感性72.9%、特异性为86.7%。结论抗环瓜氨酸肽抗体在类风湿性关节炎的诊断中敏感性比类风湿因子低,但特异性比类风湿因子高,抗环瓜氨酸肽抗体与类风湿因子的联合应用,更有助于类风湿性关节炎的早期诊断。 Objective to investigate the application of Anti-cyclic citrullinated peptide antibody(Anti-CCP) and rheumatoid factor (RF) in the diagnosis of rheumatoid arthritis (RA).Methods 74 patients with RA,50 patients with other diseases.Enzyme linkage immunosorbent assay was used to Anti-CCP.RF was measured by latex turbidimetric immunoassay.We analyzed the correlation between Anti-CCP titers and RF.Results the diagnostic sensitivity of RF for RA was 79.7% and the specificity was 52.7%,whereas the diagnostic sensitivity of Anti-CCP for RA was 60.8% and the specificity was 96.4%,the diagnostic sensitivity of the anti-CCP and RF for RA was 72.9% and the specificity was 86.7%.Conclusion our results suggest that the specificity of anti-CCP titers is better than RF reflect for RA but the diagnostic sensitivity of RF is better than anti-CCP for RA.To exam the anti-CCP and RF may be useful factors for early positive or negative RA patients.
