我院门诊不合理处方分析

目的审查门诊处方开具情况,找出存在的问题,及时反馈给医生,为临床合理用药提供参考。方法采用回顾性调查方法,随机抽取2009年11月～2010年11月门诊处方25378张进行审核,对其中不合理处方进行分析汇总。结果其中不合理处方462张,占总处方的1.82%。总体来讲门诊处方基本合理,但仍存在一些问题,值得临床医师和医院领导的高度重视,主要为用法用量,忽视基础疾病,重复用药,中西药联合应用不合理等方面。结论门诊药房药师要加强专业技术学习,严格把好审方关,建立合理用药的监督机制,提高药物治疗水平,促进药品的合理使用。

Objective To audit the State of the Out-patient Clinic Prescriptions in our hospital,find out the existing problems,Initiatively feedback to the doctors,to provide a reference for clinical rational use of drugs.Methods In a retrospective study,25378 prescriptions which were randomly sampled from outpatient department from November 2009 to November 2010 were audited.Results Among 462 prescriptions of irrational use of drugs,in the total was 1.82%.But in general,theOut-patient Prescriptions in our hospital were reasonable in use basically,but there still exist problems,which worthy of clinical doctors and hospital s leaders pay close attention to,It concludes wrong dosage and usage,neglecting physical illnesses,redundant medication,the Chinese and western drugs apply unreasonable jointly and so on.Conclusion The pharmacists must strengthen professional？study,by strictly enforcing the inspection of prescriptions,establish a rational use of drugs supervision mechanism.Improving the level usage of drug[1],that promote the drugs to rational use.
