凉血化瘀通络法治疗小儿过敏性紫癜肾炎临床疗效及病理类型关系

Therapeutic effect of cooling blood and removing stasis and dredging collaterals therapy on children with henoch-schonlein nephritis

目的:观察凉血化瘀通络法治疗小儿过敏性紫癜肾炎的临床疗效,探讨肾活检病理类型与临床疗效之间的规律。方法:选取小儿过敏性紫癜性肾炎住院患儿80例,随机分为对照组(39例)和治疗组(41例),对照组给予强的松、雷公藤多苷片、潘生丁、维生素C片常规治疗,治疗组患者予雷公藤多苷片、潘生丁、维生素C片并加用具有凉血化瘀通络功效的丹芍颗粒Ⅱ号治疗,2个月为1个疗程,有效病例观察3个疗程。统计分析两组在总体疗效、治疗前后临床症状体征积分变化等方面的差异,并观察不同肾活检病理类型患者疗效差异。结果:治疗组总体疗效优于对照组(P<0.05),且治疗组在改善紫癜性肾炎患者症状、体征方面2周时疗效优于对照组(P=0.001),肾活检病理类型分度轻者疗效更好。结论:凉血化瘀通络法治疗小儿过敏性紫癜肾炎疗效肯定,尤其在肾活检病理类型较轻患者中具有良好疗效。

Objective： To observe the clinical effect of the cooling blood and removing stasis and dredging collaterals therapy in treating children with Henoch-Schonlein nephritis,and to discuss the relationship between renalbiopsy pathological type and clinical effect.Methods： 80 cases of in-patient that met the criterion of accumulation of Henoch-Schonlein nephritis were selected,all subjects were divided randomly into two groups,controlled group（39 cases） and treatment group（41 cases）.The control group was treated with Prednisone,Tripterygium wilfordii Tabllets,Dipyridamole,Vitamin C Tablets,While the treatment group was given Tripterygium wilfordii Tabllets,Dipyridamole,Vitamin C Tablets,plus Danshao Granule No.Ⅱ,whose effect lies in cooling blood and removing stasis and dredging collaterals,2 months as one course of treatment and we observed 3 courses.We statistically analyzed the differences of the two groups on general efficacy and symptom integral,then,we observed the differences of clinical effects among different renal biopsy pathological types.Results： The general efficacy of the treatment group was better than that of the controlled group,the improvements of related symptoms in the test group were better than that of the control group.Better clinical effects were obtained in children with mild renal biopsy pathological types.Conclusion： The effects of the cooling blood and removing stasis and dredging collaterals therapy on treating children with Henoch-Schonlein nephritis is definite,especially in children with mild renal biopsy pathological types.
