中药治疗多囊卵巢综合征文献的统计学分析

Statistical Analysis of Chinese Herbal Application in Polycystic Ovarian Syndrome Treatments by Collecting Clinical Information

目的收集1984-2009年间在国内公开发表的关于中药治疗多囊卵巢综合征(PCOS)的临床研究文献,建立关联型数据库,对中药在治疗PCOS中的应用状况进行分析总结,找出常用并且与较高治愈率相关的中药。方法文献检索采用PUBMED、中国期刊全文数据库和万方数据库。重要相关文献的参考文献用人工检索。所用关键词为"多囊卵巢综合征"、"多囊卵巢"、"中药"、"中医"、"传统中医"及"中西医结合",所选文献为经连续3个月或3个月经周期治疗后以怀孕或正常排卵为主要疗效指标的临床试验及病历报告。中药的功效及分类根据英文版教科书《中药学》输入数据库。采集的数据包括中药的使用频次、使用方法及相关治疗效果等。利用Microsoft SQL Server和Microsoft Excel对数据进行排序和挖掘,从而找出最常用并且与较高治愈率最相关的中药加以总结。结果在所查到的125篇相关文献中,共有8 150例患者接受中药、中西药结合(西药作为对照)治疗。共涉及30个类别的178味中药。经数据分析发现,有16味中药是常用并与较高治愈率相关的。结论从178味中药中选出了治疗PCOS的常用且与较高治愈率相关的16味中药,它们主要为活血药和补阳药。

Objective To exam the application status, treatment strategies and clinical outcomes of Chinese herbal medicine in PCOS treatments by establishing and analyzing a relational database that collects information from clinical trials and case serial studies published between 1984 and 2009. Methods Publications were searched and retrieved from PUBMED, CNKI and Wanfang Data （1984-2009）. Reference lists of each primary study were hand-searched. Clinical trials and case serial studies using pregnancy rate or confirmed ovulation as primary outcome measures and with treatment durations of at least three months or three menstrual cycles were selected for the study. Microsoft SQL Server program and Microsoft Excel were used as data collection and database management tools. Functions and action sections of Chinese herbs were entered into the database according to Materia Medical, a comprehensive and recognized Chinese herbal medicine textbook. Herbal usage frequency, treatment strategies and associated treatment outcomes were analyzed. Most frequently used herbs also associated with higher cure rates were screened by data sorting and data mining using Microsoft SQL Server program and Microsoft Excel. Results In 125 relevant publications, 8 150 patients were treated with Chinese herbs alone, with integral Chinese herbs and western medicine, or with western medicine alone as controls. A total of 178 herbs from 30 action sections were used by different practitioners. By sorting the database and data mining using Microsoft Association Rules algorithm, 16 of the 178 herbs were found to be the most frequently used herbs also associated with higher cure rates in treating PCOS. Conclusion 16 kinds of most frequently used herbs that were also associated with higher cure rates in treating PCOS were found, which were mostly from herbs that promote circulation and remove stasis, herbs that tonify Yang.
