腹膜透析一体化护理的临床效果

Effects of Integrated Nursing Model in Peritoneal Dialysis Patients

目的探讨腹膜透析一体化护理模式对腹膜透析患者的临床效果。方法采用便利抽样法选取将2007年1-12月在南京军区南京总医院进行腹膜透析的48例患者为对照组,使用传统健康教育模式;同法选择2009年1-12月在同院行腹膜透析的97例患者为试验组,使用一体化健康教育模式。比较两组患者行腹膜透析1年后的营养状态、透析充分性、并发症发生率及社会回归率。结果透析1年后,试验组患者平均血红蛋白含量为(89.5±18.7)g/L,高于对照组的(82.2±17.8)g/L,差异有统计学意义(t=-2.235,P<0.05)。试验组患者尿素清除指数、肌酐清除率与对照组相比,差异有统计学意义(P<0.05或P<0.01)。试验组患者腹膜炎发生率低于对照组,差异有统计学意义(P<0.01)。试验组患者的社会回归率为59.90%,高于对照组的35.40%,差异有统计学意义(χ2=7.642,P<0.01)。结论腹膜透析一体化护理模式是一种有效的、可操作性强的腹透患者护理模式。

Objective To discuss the effects of integrated nursing model in the peritoneal dialysis patients.Methods With convenience sampling method,48 peritoneal dialysis patients in the hospital from January to December 2007 were selected as control group to undergo the routine health education model.In the same way,97 new peritoneal dialysis patients admitted to our hospital in the year 2009 were chosen as experimental group,receiving a integrated health education model.Comparisons were conducted on the nutrition status,complications during dialysis,dialysis adequacy and rate of social return were compared between the two groups one year after the peritoneal dialysis.Results After 1 year of dialysis,the average hemoglobin content of the experimental group[（89.5±18.7）g/L] was significantly higher than that [（82.2±17.8）g]/L of the control group（t=-2.235,P0.05）.Significant differences were observed on creatinine clearance rate（CCR）（P0.05 or P0.01）.The peritonitis rate of experimental group was significantly lower than that of the control group（P0.01）.The social regression rate was significantly higher in the experimental group（59.90%） than that in the control group（35.40%）（χ^2= 7.642,P0.01）.Conclusion The integrated nursing model serves as an effective and practical nursing model for peritoneal dialysis patients.
