无创正压通气联合舒利迭治疗COPD急性加重期疗效观察

The efficacy of noninvasive positive pressure ventilation combined with Seretide on acute exacerbation of chronic obstructive pulmonary disease

目的探讨无创正压通气(NPPV)联合舒利迭对AECOPD患者的治疗作用。方法选取AECOPD患者63例,随机分成三组,对照组1为常规抗炎、平喘及吸氧治疗,对照组2在对照组1基础上只给予吸入舒利迭,治疗组在常规抗炎、平喘及吸氧基础上,予无创呼吸机辅助呼吸,同时吸入舒利迭,观察不同时间治疗前后RR、HR及血气变化。结果在24 h和48 h,NPPV联合舒利迭治疗与单一采用舒利迭治疗比较,对RR、HR、PaO2及PH值的改善均有统计学意义(P<0.05);NPPV联合舒利迭与单一应用舒利迭治疗比常规治疗各项指标改善均具有统计学意义(P<0.05)。结论 NPPV联合舒利迭治疗AECOPD较单一应用舒利迭能快速缓解患者症状及呼吸生理,值得临床早期应用。

Objective To investigate the effects of noninvasive positive pressure ventilation（NPPV） combined with Seretide on acute exacerbation of chronic obstructive pulmonary disease（AECOPD） patients.Methods63 AECOPD patients were randomly divided into three groups,the control group 1 was treated by conventional therapy only,the control group 2 was given inhaled seretide on this basis and the treatment group was even more combined with NPPV.Obsrving the changes of RR、HR and arterial blood analysis in different time during treatment.ResultsAt the 24th and 48th hour after treatment,the levels of RR、HR、PaO2 and PH showed an obvious improvement between the treatment group and control group 2（P0.05）.And compared with control group 1,there were significantly different between two comparison（all P0.05）.ConclusionNPPV combined with seretide is a effective treatment for AECOPD and had superior to single drug group,and worth using early in clinic.
