原发性高血压患者血压昼夜节律变化与血浆脂联素水平的相关性研究 被引量:4

Relationship Between Circadian Rhythm of Blood Pressure and Plasma Adiponectin Levels in Essential Hypertension Patients

摘要 [目的]探讨原发性高血压病患者24h动态血压昼夜节律变化与血浆脂联素(APN)水平的关系.[方法]原发性高血压(EH)患者224例根据24h动态血压监测(24hABPM)结果分为杓型组123例和非杓型组101例 ;取空腹静脉血,测定血浆脂联素(APN)、胰岛素(FINS)、血糖(FPG),测量身高、体重,计算胰岛素抵抗指数(IRI)、体重指数(BMI),并进行分析、比较.[结果]①杓型高血压组血浆APN水平显著高于非杓型高血压组(P<0.01).②杓型高血压组患者BMI、IRI低于非杓型高血压组(P<0.05).③血浆APN水平与性别、收缩压(SBP)、BMI、IRI呈负相关(P〈0.05或0.01).[结论]血浆APN水平与血压昼夜节律相关,非杓型高血压病患者较杓型高血压病患者血浆APN水平更低. [Objective] To explore the relationship between 24 hour ambulatory blood pressure variability and plasma adiponeetin levels in essential hypertension(EH) patients. [Methods] Totally 224 EH cases were divided into dipper group( n=123) and non-dipper group( n =101) according to 24-hour ambulatory blood pressure monitoring(24hABPM). Fasting venous blood was drawn to detect plasma adiponeetin, fasting insulin(FINS), fasting plasma glucose(FPG). Body height and weight were measured. Body mass index(BM1) and insulin resistance index(/RI) were calculated. [Results] The plasma adiponectin level in dipper hyperten sive group was significantly higher than that in the non dipper hypertensive group( P〈0.01). The BMI and IRI in dipper hypertensive group were lower than those in non dipper hypertensive group( P 〈0.05). Plasma adiponectin level was negatively correlated with 24 hour systolic blood pressure(SBP), BMI and IRI( P 〈0.05 or 0.01). [Conclusion]Plasma adiponectin level is correlated with circadian rhythm of blood pressure. Plasma adiponectin level in non-dipper hypertensive patients is lower than that in dipper hypertensive patients.
