多侧孔双腔双囊支气管导管在胸外科手术中的应用

Preparation and Clinical Application of Double-Lumen Double-Balloon Endobronchial Tubes(DLT) with Multi-side Holes in Thoracic Surgery

目的:验证多侧孔双腔双囊支气管导管在胸外科手术中的临床应用效果。方法:多侧孔双腔双囊支气管导管是对现有的双腔双囊支气管导管进行了改进,在双腔双囊支气管导管的次支气管导管侧增设多个侧孔开口,同时将总气管套囊上移固定。选择胸外科患者50例,应用新型的多侧孔双腔双囊支气管导管行全麻气管插管。结果:气管插管成功率为100%。全部病例术中氧饱和度均高于98%,双肺隔离均成功,满足手术要求,无并发症发生。结论:多侧孔双腔双囊支气管导管克服了现有双腔双囊支气管导管的缺陷,使用安全、更方便,是一款比较理想的双腔双囊支气管导管。

Objective：To verify the feasibility of clinical application of double-lumen double-balloon endobronchial tubes with multi-side holes（DDLT） in thoracic surgery.Methods：DDLT is an improvement on the traditional double-lumen endobronchial tubes（DLT） through a upward displacement of trachea＇s air bag and through increase of multi-side holes on the main bronchi tube.Endotracheal intubation were performed on 50 patients in thoracic surgery under general anesthesia with the new-style DDLT.Results： The successful rate was 100%.In all of the patients,the pulse oxygen saturation was over 98%,moreover,lung separation was excellent and meets the requirement of one-lung ventilation.Conclusions： The new-style DDLT,which overcomes shortcomings of the traditional,is a more ideal DLT as it is more swift,more secure and more convenient for meeting the requirement of one-lung ventilation.
