350例女性患者平板运动试验与冠状动脉造影结果分析

Comparison of results of treadmill exercise test with coronary angiography in 350 female patients

目的探讨平板运动试验(TET)对诊断女性冠心病(CHD)的预测价值。方法选择临床可疑女性CHD患者350例1,周内相继完善TET与选择性冠状动脉造影(CAG)检查。将TET的结果与CAG的结果进行分析。结果 TET检出女性CHD的敏感性为78.2%,特异性为71.1%,阳性预测值71.9%,阴性预测值77.6%,预测准确性74.6%,假阳性率为28.9%,假阴性率为21.8%。TET阳性率与冠脉病变支数相关;TET中ST段下移程度、出现时间及持续时间与冠脉狭窄程度相关。结论 TET是目前诊断女性CHD较理想的无创性检查方法,并可估测冠脉病变程度。

Objective To explore the prediction value of coronary angiography（CAG） and treadmill exercise test（TET） in diagnosis of coronary heart disease in female patients.To elucidate the value of treadmill exercise testing in diagnosis of coronary artery disease（CHD） and the relationship between treadmill exercise test and pathogenic process of coronary artery.Methods Three hundred and fifty female patients with suspected CHD in clinic were examined with treadmill exercise test and coronary angiography within one week.The results of treadmill exercise test were compared with those of CAG.Results The rates of sensitivity,specificity,positive predictive value and negative predictive value,predictive accuracy,false positive rate and false negative rate were 78.2%,71.1%,71.9%,77.6%,74.6%,28.9% and 21.8% respectively.The positive rate of treadmill exercise test was correlated to pathological changes in coronary artery.Conclusion Treadmill exercise test is one of non-invasive methods in diagnosis of CHD in female patients.It is simple,less expensive,practical and fit for application in clinic.
