硬膜外人参皂苷药囊电极对兔蛛网膜下腔出血脑血流动力学影响及意义的实验性研究 被引量:1

Effects of Epidural Electrodes with Ginsenoside Capsules on Bloodstream Dynamics Phenomenon in Rabbits after Subarachnoid Hemorrhage and It's Significience

摘要 目的:探讨硬膜外人参皂苷药囊电极对兔蛛网膜下腔出血脑血流动力学的影响及意义。方法:健康新西兰大白兔32只,随机分成人参皂苷药囊电极治疗组和尼莫地平对照组,采用兔视交叉注血法制造蛛网膜下腔出血模型,应用64排螺旋CT灌注成像(CT perfusion,CTP)分别测量注血前至14天治疗组和对照组的脑灌注压(CPP),脑血流量(CBF)、脑血容量(CBV)、达峰时间(TPP)及大脑前动脉、大脑中动脉、基底动脉、失状窦、直窦和大脑大静脉直径,并比较两组之间的参数。结果:注血第2~14天治疗组的平均ICP明显低于对照组,而CPP明显高于对照组,治疗组第4~12天的CBF、第2~12天的CBV明显大于对照组,而第4~12天的TPP均小于对照组(P<0.05)。蛛网膜下腔出血第4~14天治疗组上失状窦平均直径均大于对照组,蛛网膜下腔出血第2~14天:治疗组直窦和大脑大静脉直径均大于对照组(P<0.05)。结论:人参皂苷药囊电极与尼莫地平地比较,明显的降低了SAH大白兔ICP和TPP,并增加了引流静脉直径、CPP、CBF和CBV,为蛛网膜下腔出血的治疗提供了一条新的参考途径。 Objective:To study the effects of epidural electrode with ginsenoside capsule on bloodstream dynamics phenomenon in rabbits after subarachnoid hemorrhage and it's significience.Methods: 32 healthy New Zealand white rabbits were randomly divided into electrode treatment group and control group,suprachiasmatic blood injection technique was used for producing subarachnoid hemorrhage model.Cerebral perfusion pressure(CPP)、cerebral blood flow(CBF)、 cerebral blood volume(CBV) 、 peak time(TPP) and diameters of anterior cerebral artery,middle cerebral artery,basilar artery,superior sinus,straight sinus and great cerebral vein were measured with 64-slice spiral CT perfusion imaging(CT perfusion,CTP)technique before blood injection to 14th day after injection and compare the parameters between the two groups.Results: The mean ICP was significantly lower than the control group while the CPP was significantly higher in the treatment group at 2th to 14th day after injection.The CBF and CBV were significantly higher than the control group perspectively at 4th and 2th day until 12th day after injection while the TPP is less than the control group(P0.05)at 4th to 12th day.The average diameters of superior sinus were significiently grater than control group at 4th to 12th day while those of straight sinus and great cerebral veins were grater than control group(P0.05)at 2th to 12th day.Conclusions: Epidural electrode with ginsenoside capsule compared with nimodipine significantly reduces the ICP and TPP,and increase the CPP、CBF 、CBV and the drainage vein diameters in the treatment for rabbits after subarachnoid hemorrhage and offers a new referential method.
