氨酚羟考酮联合加巴喷丁治疗带状疱疹后遗神经痛的临床观察

Safety and efficacy of oral Oxycodone/Acetaminophen combined with Gabapentin in treatment of postherpetic neuralgia

目的观察氨酚羟考酮联合加巴喷丁治疗带状疱疹后遗神经痛(PHN)的疗效和安全性。方法 66名PHN患者随机分为氨酚羟考酮组(A组,n=22)、加巴喷丁组(B组,n=22)和联合治疗组(C组,n=22),A组给予氨酚羟考酮片0.99/d、B组给予加巴喷丁胶囊1200mg/d和C组给予氨酚羟考酮片0.99/d+加巴喷丁胶囊600 mg/d,均治疗4周。观察治疗前后疼痛强度(PI)变化,疼痛缓解度(PAR),镇痛有效率及不良反应。结果 3组服药后NRS评分均明显降低(P<0.05),A、B组间比较无显著差异(P>0.05),C组较A、B组评分降低(P<0.05);C组的镇痛显效率高于A、B两组(P<0.05);三组药物的副作用主要表现为头晕、嗜睡、恶心、呕吐,副作用的总发生率无显著差异(P>0.05)。结论氨酚羟考酮联合加巴喷丁治疗PHN,镇痛效果良好,安全可靠,是PHN止痛可供选择的措施之一。

【Objective】 To evaluate the safety and efficacy of oral Oxycodone/ Acetaminophen（tylox） combined with Gabapentin in treatment of postherpetic neuralgia（PHN）.【Methods】 66 adult patients with PHN were randomly divided into tylox group（group A,n =22）,Gabapentin group（group B,n =22） and combination group（group C,n =22）.The patients of group A were took tylox table 0.99 g per day.The patients of group B were took Gabapentin capsule 1 200 mg per day.The patients of group C were took tylox table 0.99 g and Gabapentin capsule 600 mg per day.All the groups were treated for 28 days.The numeric rating scales（NRS） changes,efficacy rate of relieving pain and the side effects were observed before and after treatment.【Results】 The NRS value of three groups had obviously decreased after treatment comparing with before treatment（P 0.05）,and the value of group C was lower than that of group A and B（P 0.05）.The efficacy rate in group C was higher than that in group A and B（P 0.05）.The common adverse events were dizziness,somnolence and vomiting.The overall incidence of side effects were not significant difference in three groups（P 0.05）.【Conclusion】 Tylox combined with Gabapentin is an effective and safe way in treatment of postherpeic neuralgia.
