子宫颈癌的MRI诊断(附45例病例)

Diagnostic value of MRI in cervical cancer

目的探讨MRI在子宫颈癌诊断、治疗及预后中的临床意义。方法回顾性分析河南省开封市第一人民医院经病理证实的45例子宫颈癌患者MRI表现、诊断及分期,将MRI诊断结果与病理诊断比较。结果 40例MRI发现了病灶。在T1W I宫颈癌与周围宫颈组织相比呈等信号,在T2W I,宫颈癌呈稍高于正常宫颈组织。具有宫旁浸润时,显示为癌肿周围正常低信号带中断或消失,Gd2DTPA增强后肿瘤组织不同程度强化。28例经单纯放疗,9例肿瘤信号完全消失,17例肿瘤不同程度缩小,局部T2W I序列呈低信号,出现不同程度无强化区,2例无变化。结论 MRI能准确显示子宫肿瘤病灶,并多方位观察癌肿侵犯范围,分期客观准确,对临床制订治疗方案及预后评估具有重要指导意义。

Objective To discuss the clinical significance of MRI in cervical carcinoma of diagnosis,treatment and prognosis.Methods Retrospectively analysis through the 45 cervical cancer cases by pathology which were confirmed in our hospital with their MRI manifestations,diagnosis and staging.Compared the MRI diagnosis with the pathological diagnosis.Results MRI lesions were found in 40 patients.T1WI cervical cancer in cervical tissue compared with the surrounding positive signal,in the T2WI,cervical cancer was slightly higher than the normal cervical tissue.Parametrium tumor with infiltration,the tumor showed low signal with surrounding normal interrupted or disappear,after enhance Gd2DTPA tumor tissue enhanced to varying degrees.28 cases by radiotherapy alone.5 patients of tumor signal completely disappeared.17 patients’ tumor reduced in different degrees,with low-signal in partial sequence of T 2W I and no enhancement area with varying degrees.And 6 patients no change.Conclusions MRI can accurately show the uterine tumor lesions,observe the violations of the scope of cancer in many sides,phases the clinical treatment objectively and accurately.It has great guiding significance in the clinical treatment and prognosis assessment.
