慢乙肝携带者感染麻疹HBV变化及其与细胞因子关系

The association of HBV-DNA change with cytokines levels in chronic HBV carriers superinfected with measles virus

目的观察慢性HBV携带者重叠感染麻疹后外周血乙型肝炎病毒（HBV）DNA载量、细胞因子变化及两者关系。方法23例重叠感染麻疹慢性HBV携带者，27例慢性HBV携带者和27例麻疹患者使用PCR检测HBV—DNA载量，Luminex液相蛋白芯片检测7种细胞因子IFN—r、TNF—α、IL-2，4，5，10及RANTES水平。结果（1）重叠感染组外周血HBV—DNA载量显著低于慢性HBV组垆〈0．05）；（2）三组血清IFN—r、TNF—α、IL-4及IL-5水平差异有显著性（P〈0．05），重叠感染组IFN—r、TNF—α水平显著高于其余两组f21．25±7．48VS15．85±4．45、18．25±5．56，58．74±16．42vs25．12±12．72、38．35±10．48）pg／ml（P〈0．05），IL4、IL-5水平则显著低于其余两组（9．52±3．87vs20．82±7．25、15．45±6．94，12．26±5．73VS18．57±6．75、16．25±3．77）pg/ml（P〈0．05），三组血清IL-2、RANTES及IL-10水平无差别（P〉0．05）；（3）重叠感染组和慢性HBV组HBV—DNA载量与血清IL-4，IL-5水平均呈正相关，与IFN—r、TNF—α水平均呈负相关（P〈0．05）。结论麻疹病毒对慢乙肝携带者血清HBV有一定的抑制作用，这可能与Th1利细胞应答增强、Th2型细胞应答减弱有关。

Objective To investigate the HBV-DNA changes in HBV carriers superinfeeted with measles virus and its relation with cytokines. Methods HBV-DNA loads were detected by PCR in 23 chronic HBV carriers superinfected with measles virus （superinfection group）, 27 chronic HBV carriers （HBV group）, and 27 measles patienls （measles group）. Serum levels of cytokines （IL-2, 4, 5, and 10, IFN- r, TNF-α, and RANTES） were measured by Luminex Liquichip technology. Results The HBV-DNA loads were significantly lower in superinfection group than in HBV group （P〈0.05）. There were significant differences in levels of IL-4, IL-5, IFN-r, and TNF-α between the three groups （P〈0.05）; but there were no significant differences in IL-2, RANTES, and IL-10 （P〉0.05）. The change of HBV-DNA loads were negatively related with levels of IFN-r and TNF-α, but positively related with levels of IL-4 and IL-5 （P〈0. 05）. Conclusions The elevated levels of IFN-r and TNF-α and the decreased levels of IL-4 and IL-5 in chronic HBV carriers superinfeeted with measles vires can probably lead to the inhibition of HBV-DNA replication.
