2005～2009年沅陵县结核病痰检资料分析

Data Analysis on Tuberculosis Sputum Diagnosis in Yuanling County,2005-2009

[目的]了解结核病痰涂片镜检结果,以便总结经验,采取措施提高结核病痰检阳性率。[方法]对2005~2009年到沅陵县疾病预防控制中心就诊的可疑结核病症状者、可疑肺结核病人及进行规范治疗病人的痰检结果进行分析。[结果]2005~2009年累计,初诊有可疑症状者平均查痰率为74.01%,平均初诊涂阳率为35.25%,各年阳性率为30.19%~40.21%（P〈0.01）。痰涂片阳性率,初诊可疑者为16.19%,随防病人为3.64%（P〈0.01）;血性样痰、脓性痰阳性率分别为23.66%、21.82%,干酪样痰、脓粘液痰、水样痰分别为8.18%、1.20%、0.72%（P〈0.01）;晨痰、即时痰、夜间痰分别为12.74%、9.34%、7.39%（P〈0.01）。[结论]沅陵县初诊涂阳率处于一般水平;不同病人、不同性状、不同时间采集的痰标本涂阳率不同。

[Objective] To understand the microscopic examination result of the tuberculosis sputum smear so as to summarize the experience and take measures to improve the positive rate of tuberculosis sputum.[Methods]Analysis was made on the sputum from the patients of suspected tuberculosis symptoms and suspected tuberculosis,who came to Center for Disease Control and Prevention in Yuanling county,2005-2009.[Results]74.01% of patients with the suspected tuberculosis symptoms had their sputum checked up in their first visits to the hospital,the average smear positive rate was 35.25%,the positive rate of all age groups was 30.19%-40.21%（P0.01）,the sputum smear positive rate of the suspected patients was 16.19%,and 3.64%（P0.01） of the suspected patients followed up their visits to the hospital.The congestive sputum rate was 23.66%,purulent sputum rate was 21.82%,caseating sputum rate was 1.20%,the mucopurulent sputum rate was 0.72%（P0.01）,morning sputum rate was 12.74%,spot sputum rate was 9.34%,and night sputum rate was 7.39%（P0.01）.[Conclusion]The smear positive rate of patients in their first visits to the hospital is at the average level,and the smear positive rate of the sputum samples varies from patients,symptoms and time.
