慢性重症肝炎并发肝肾综合征30例临床分析

Clinical Analysis of 30 Case of Chronic Severe Hepatitis Complicated with Hepatorenal Syndrome

目的探讨慢性重症肝炎并发肝肾综合征患者的诊断、治疗。方法回顾性分析我院收治的30例慢性重症肝炎并发肝肾综合征患者临床特点、实验室检查、治疗及预后情况。结果：本组患者住院期间儿例死亡（36．7％），9例病危自动出院，10例好转出院。出院患者4例生存超过1a，2例生存时间超过2a。结论：提高对慢性重症肝炎合并肝肾综合征预防及早期诊断有很高的临床使用价值，对慢性重症肝炎合并肝肾综合征的治疗主要针对肝功能的损害、有效血容量的减少、肾内血循环的改变及内毒素血症的控制等多个环节进行。

Objective To investigate the chronic severe hepatitis complicated by hepatorenal syndrome diagnosis and treatment. Methods A retrospective analysis of our hospital, 30 patients with chronic severe hepatitis complicated by hepatorenal syndrome in patients with clinical features, laboratory findings, treatment and prognosis. Results 11 patients died during hospitalization （36.7%）, 9 patients discharged from hospital in critical condition, 12 were improved and discharged. 4 patients were discharged more than 1 year survival, 2 patients survived for more than 2 years. Conclusion The increase in chronic severe hepatitis complicated by hepatorenal syndrome prevention and early diagnosis of high clinical value of chronic severe hepatitis complicated with Hepatorenal syndrome aimed at damage to liver function, the effective reduction of blood volume, renal changes in blood circulation and the control of endotoxemia and other parts to this.
