肾上腺钝性损伤的CT表现

CT manifestations of adrenal blunt injury

目的探讨肾上腺钝性损伤的CT表现及其诊断价值。方法回顾性分析经临床或随访证实的肾上腺损伤27例的CT检查资料,其中男25例,女2例,均行CT常规扫描,其中8例行CT增强扫描。结果全部为单侧肾上腺损伤,分为挫伤型(9例)及血肿型(18例),挫伤型CT主要表现为肾上腺肢的肿胀、增粗;血肿型表现为肾上腺区类圆形或卵圆形血肿。两种类型伴有或不伴有肾上腺周围脂肪内出血。均合并胸腹部其他组织、脏器损伤,无单纯肾上腺损伤。定期复查肾上腺血肿密度逐渐降低,肿胀减轻,肾上腺形态逐渐恢复正常。结论 CT检查能直观反映肾上腺损伤的类型及程度,能够反映肾上腺损伤的病理改变,对肾上腺损伤的诊断具有重要价值。

Objective To investigate the CT performance of adrenal blunt injury and its value.Methods Retrospective analysis of follow-up confirmed by clinical or injury of 27 cases of adrenal CT examination data,including 25 males and2 females,underwent routine CT scan,8 of routine enhanced CT scanning.Results Its were all unilateral adrenal injury,Divides into the bruise(9 cases) and the haematoma(18 cases) bruise-type CT mainly adrenal limb swelling,thickening;hematoma adrenal gland showed round or oval hematoma.With or without two types of fat around the adrenal hemorrhage.All other organizations combined chest and abdomen,organ damage,no simple adrenal injury.Regularly review the density decreased adrenal hematoma,swelling to reduce,gradually returning to normal adrenal shape.Conclusion CT examination can directly reflect the type and extent of adrenal injury,can reflect the pathological changes of adrenal injury,the diagnosis of adrenal injury of great value.
