吉西他滨联合顺铂在非小细胞肺癌术前新辅助化疗效果和耐受性初探

Comparative study on gemcitabine plus cisplatin as preoperative neoadjuvant chemotherapy for no-small cell lung cancer

目的观察吉西他滨(GEM)联合顺铂(DDP)和长春瑞滨(NVB)联合DDP用于Ⅲ期非小细胞肺癌(NSCLC)术前新辅助化疗的效果及耐受性。方法 27例Ⅲ期NSCLC手术患者,术前接受新辅助化疗,其中吉西他滨组12例接受GEM+DDP方案,与同期接受诺维苯NVB+DDP方案(NP组)15例进行比较。结果 GEM组,CR1例,PR4例,总有效率41.7%;NVB组,CR2例,PR4例,总有效率40.0%,两组有效率比较差异无统计学意义(P>0.05);化疗后肿瘤分期下降两组分别为:GEM组33.3%(4/12),NVB组40.0%(6/15),两组比较差异无统计学意义(P>0.05)。结论吉西他滨联合顺铂方案用于NSCLC术前新辅助化疗效果肯定,其均可作为NSCLC术前新辅助化疗的可选择方案。

Objective To evaluate efficacy and toxicity of gemcitabine（GEM） plus cisplatin（DDP） regimen in preoperative adjuvant chemotherapy in patients with Ⅲ stages NSCLC.Methods In all of the 27 patients with stage Ⅲ NSCLC,12 cases received GEM ＋DDP regimen,while the other 15 cases received NVB＋DDP（NP） regimen.The therapeutic efficacy and toxicity of GEM ＋DDP regimen were observed and with NP regimen.Results The overall response rate was 41.7% for GEM group with 1 complete response（CR） and 4 partial response（PR）,and 40.0% in the NVB group with 2 cases CR and 4 cases PR（P0.05）.There were 33.3%,down after preoperative chemotherapy in Alimta group,and 40.0%in NP group（P0.05）.The main toxicity is mylosuppression,there was no significant difference between the two groups（P0.05）.Conclusion GEM＋DDP regimen was a highly active regimen of the neoadjuvant chemotherapy for NSCLC,and it could be used as a regimen for the preoperative neoadjuvant chemotherapy of NSCLC.
