自体骨髓干细胞移植治疗脊髓损伤神经源性膀胱疗效观察:1例脊髓损伤患者移植前后尿动力学指标变化

摘要 目的探讨自体骨髓干细胞移植治疗脊髓损伤患者神经源性膀胱的效果。方法 脊髓损伤患者1例,于伤后13d开始给予规律自体骨髓干细胞移植治疗,共9次,检测治疗前后尿动力学指标。结果 患者伤后26d最大尿道压及尿道闭合压力明显升高,膀胱容量增大;伤后140d时患者膀胱感觉恢复,可诱发出逼尿肌自主收缩,最大压力为30cmH2O(10cmH2O=0.98kPa)。结论 在脊髓损伤后早期,规律地按疗程给予自体骨髓干细胞移植可有效地改善神经源性膀胱排尿功能障碍。 Objective To investigate the effect of autologous bone marrow derived stem cell transplantation on the neurogenic bladder of the patients with spinal card injury.Methods One patient with spinal cord injury were treated by autologous bone marrow derived stem cell transplantation for 9 times 13 day after injury.The urodynamic profile was measured after and before treatment.Results Twenty-six days after injury,maximum urethra pressure,maximal urethral closure pressure and maximum bladder capacity increased obviously.One hundred and forty days after treatment,recovery of bladder sensibility was observed in this patient.Detrusor muscle contraction could be induced,and maximum pressure could reach 30 cm H2O(10 cmH2O=0.98 kPa).Conclusion In acute period of spinal card injury,regular autologous bone marrow derived stem cell transplantation may improve neurogenic bladder symptoms effectively.
