胃癌患者幽门螺杆菌感染及与bcl-2、bad相关性研究 被引量:2

摘要 目的:通过检测凋亡相关基因bcl-2、bad在胃癌中的表达,进一步探讨幽门螺杆菌(Hp)对胃上皮细胞增殖和凋亡的影响。方法:对79例胃癌及29例慢性浅表性胃炎受试者应用快速尿素酶法及改良吉姆萨染色法检测Hp感染情况,用免疫组化SABC法检测胃黏膜中bcl-2、bad蛋白的表达。结果:胃癌组和慢性浅表性胃炎组中Hp阳性率分别为69.6%(55/79)和48.3%(14/29)(P<0.05)。胃癌组中bcl-2蛋白表达阳性率为65.8%(52/79),明显高于慢性浅表性胃炎组24.1%(7/29()P<0.05);胃癌组bad蛋白表达阳性率为46.8%(37/79),明显低于慢性浅表性胃炎组69.0%(20/29)(P<0.05)。胃癌组中Hp感染阳性组bcl-2蛋白表达阳性率明显高于Hp阴性组(78.2%vs 37.5%),bad蛋白表达阳性率明显低于Hp阴性组(38.2%vs 66.7%),差异有统计学意义(P<0.05);胃癌组bcl-2、bad蛋白的表达与肿瘤的分化程度及浸润深度有关,而与淋巴结转移无关。结论:Hp感染可能通过调节凋亡相关基因蛋白的表达使细胞增殖和凋亡异常而参与胃癌的发生发展过程。 Objective: To explore the influence of Helicobacter pylori(Hp) on gastric epithelial cell proliferation and apoptosis.Methods: Hp was assessed by using rapid urease staining test combined with modified Giemsa staining test in 79 cases of gastric carcinoma and 29 cases of chronic gastritis.The expressions of bcl-2 and bad proteins in gastric mucosa were detected by immunohistochemistry(SABC) staining.Results: The positive rates of Hp in gastric carcinoma group and chronic gastritis group were 69.6%(55/79) and 48.3%(14/29)(P0.05).The positive rate of bcl-2 protein in gastric carcinoma group was 65.8%(52/79),which was significantly higher than that in chronic gastritis group 24.1%(7/29)(P0.05);The positive rate of bad protein in gastric carcinoma group was 46.8%(37/79),which was significantly lower than that in chronic gastritis 69.0%(20/29)(P0.05).In gastric carcinoma group,the positive rate of bcl-2 protein in Hp positive group was significantly higher than that in Hp negative group(78.2% vs 37.5%)(P0.05).The positive rate of bad protein in Hp positive group significantly lower than that in Hp negative group(38.2% vs 66.7%)(P0.05).The expression of bcl-2 and bad proteins were correlated with gastric carcinoma differentiation grade and infiltration range(P0.05),but not related with lymph node metastasis(P0.05).Conclusion:Hp infection may participate the development of gastric carcinoma by regulating the expression of apoptosis relate gene proteins which makes cell proliferation and apoptosis abnomal.
