胚胎着床前期子宫内膜血流与IVF-ET治疗的关系 被引量:7

摘要 目的:探讨着床前期彩色多普勒超声子宫内膜血流检测与体外受精-胚胎移植(IVF-ET)治疗的关系。方法:选择行IVF-ET患者134例,检测自然周期排卵后第3天和超促排周期取卵后第3天(ET日)的子宫内膜血流状态和参数。结果:超促排周期Ⅲ级组的妊娠率和胚胎种植率(40%,13.7%)明显高于Ⅰ级(10%,4%)、Ⅱ级(29.41%,9.3%)组,差异有统计学意义(P<0.05)。超促排周期妊娠组子宫内膜血流参数阻力指数(RI)、搏动指数(PI)、收缩期峰值流速/平均流速比值(S/D)明显高于非妊娠组,差异有统计学意义(P<0.05)。妊娠组与非妊娠组自然周期血流状态差异有统计学意义(P<0.05)。结论:子宫内膜血流状态和血流参数RI、PI、S/D值可作为IVF-ET结局的预测指标。自然周期血流丰富可能提示较好的子宫内膜容受性。 Objective: To investigate the relationship between endometrial blood flow measured by color Doppler on pre-nidation and in vitro fertilization and embryos transfer cycle(IVF-ET).Methods: Data were obtained a total 134 women undergoing IVF-ET.The endometrial thickness,blood flow status dates were detected by transvaginal ultrasonography on pre-implantation which include the third day after ovulation before IVF-ET cycles and the day of embryos transfer during IVF-ET cycles.Results: The clinical pregnancy rate and the implantation rate were significantly higher in the Group Ⅲ(40%,13.7%) than in the GroupⅠ(10%,4%)and GroupⅡ(29.41%,9.3%).The pulsatility index(PI),resistance index(RI),and S/D ratio on ET in pregnancy group were higher than in non-pregnancy group(P0.05).In natural cycles,there was significant difference between the endometrial blood status of the pregnancy group and the non-pregancy group(P0.05).Conclusion: Endometrial perforating blood flow and RI,PI,S/D values can be as a forecast level of outcomes of IVF-ET.Richer of endometrial blood flow in natural cycle may hint better endometrial receptivity.
