104例浸润性微乳头状癌分析

Invasive Micro-papillary Carcinoma—analysis of 104 patients

目的:探讨浸润性微乳头状癌(invasive micropapillary carcinoma,IMPC)的临床病理特征、诊断、综合治疗和预后。方法:对我院2004年10月—2009年3月收治的104例IMPC患者进行回顾性分析总结。结果:104例IMPC患者无特殊的症状和体征,占同期全部乳腺癌9 364例的1.11%。其淋巴结转移率77.0%及转移个数(平均9.65枚)均明显高于浸润性导管癌(41.0%,平均3.5枚),P<0.05。随访至2009年7月初,3年生存率75.0%,2年85.2%,1年96.3%;无瘤生存率3年70.8%,2年81.5%,1年95.0%。结论:IMPC是一种近几年才受到重视的乳腺癌类型,具有不可忽视的发病率、淋巴管侵袭力强、淋巴结转移率高以及预后差的生物学特征。

Objective：To observe the clinical expressions of pathology of invasive micro-papillary carcinoma（IMPC）,including diagnosis,therapy and prognosis.Methods：Pathological documents of 104 patients with IMPC,who were admitted from October 2004 to March 2009 in our hospital,were retrospectively analyzed.Results：Invasive micro-papillary carcinoma which were free of obvious signs and symptoms,accounted for 1.11 percent of 9 364 breast cancer cases in corresponding time period.The metastasis rate（77.0%） and numbers of lymph nodes（n=9.65） in IMPC was significantly higher than that in invasive ductal carcinoma（41.0%,average of 3.5）（P0.05）.After follow-up of 5 years,the 3 years,2 years and 1 year survival rate was 75.0%,85.2% and 96.3%,respectively,and there was 70.8%,81.5% and 95.0% of 3 years,2 years and 1 year disease-free survival rate respectively.Conclusions：IMPC has an incidence rate which can not be ignored,high invasiveness and metastasis of lymph,and poor prognosis.And it is a type of breast cancer attended in recent years.
