Treatment of tibial shaft and Pilon fracture with minimal invasive fixation by locking compression plate

摘要 目的探讨锁定钢板小切口治疗胫骨干部及穹窿部骨折的疗效。方法对31例胫骨干部及穹窿部骨折患者采用锁钉钢板小切口治疗。结果手术时间60~120(90±30)min。术后X线片显示骨折均达到解剖复位或接近解剖复位。31例均获得随访,时间5~24个月。根据Olerud-Molander评分系统评定:95~100(97.25±2.75)分,优24例,良5例,中1例,差1例,优良率达到93.55%。结论锁定钢板经皮小切口治疗胫骨骨折,固定方法简单,钢板把持力可靠,可早期恢复关节活动,临床效果好。 Objective To analyze the effect of locking compression plate( LCP) by minimally invasive plate osteosynthesis(MIPO) in the treatment of tibial shaft fracture and Pilon fracture. Methods 31 cases of tibial shall and Pilon fractures were fixed by LCP with MIPO. Results Operation time was 60 - 120(90±30) rain. Postoperative roentgenographies proved that all the cases got anatomical reduction. All the 31 cases had been followed up for 5 to 24 months. According to Olerud-Molander ankle scoring system,all the scores were between 95 - 100 (97.25 ± 2.75 ). The results were excellent in 24 cases,good in 5,fair in 1 and poor in 1. The rate of excellent and good was 93.55%. Conclusions It is a simple operation in the treatment of tibial fracture with LCP by MIPO. LCP has reliable pull-out strength. It can permit early functional exercises of the joint, with which good result is anticipated.
