同种原位肝移植治疗不能切除的肝尾叶肝癌

ORTOHOTPIC LIVER TRANSPLANTAION FOR UNRESECTABLE LIVER CANCER IN CAUDATE LOBE

目的 探讨肝癌肝移植的适应证、手术技术及围手术期处理特点。方法 对１ 例男性，５３ 岁的不能切除的肝尾叶癌患者进行了原位肝移植。结果：移植肝活力恢复良好，虽出现呼吸功能不全、肾功不全、肺部感染、凝血障碍、顽固性胸腹水等并发症，均经及时处理得以恢复。结论 不能切除的中央型肝癌及肝癌伴有严重肝功损害者为肝移植的良好适应证，完善的手术技术及正确的围手术期处理是保证肝移植成功的关键。

bjective To investigate the indication surgical technique and perioperative management of orthotopic liver transplantation for the patients with liver cancer.Method An orthotopic liver transplantation was succussfully performed on a 53 years old man with unresectable liver cancer on caudate lobe.Result The recovery of liver graft function was better after the operation in this patient.After transplantation such complicatioins as respiretory disfunction,renal disfunction,mixed infection,acute rejection,and abdominal ascite were overcame.Conclusion It is suggested that unresectable liver cancer,and the case with liver cancer combinated of severe liver disfunction are better indication for liver transplantation.Good surgical technique and perioperative management are key points for the success of the liver transplantation.
