仰卧位微创骶骨拉力钉治疗骨盆后环损伤

With Percutaneous lliosacral Screw Fixation For Posterior Pelvis Ring Injury on Supine Position

目的评价C形臂引导下应用微创骶骨拉力钉固定骨盆后环治疗骨盆骨折的安全性及疗效。方法仰卧位应用外固定架固定前环，C臂机引导下微创骶骨拉力钉固定后环的骨盆AO分型C型骨折患者21例，并进行评价。结果术后21例均获得随访，根据Matta评定标准，本组优18例，良2例，可1例，优良率95．23％．所有患者骨折愈合，无明显疼痛，功能恢复良好。结论仰卧位微创骶骨拉力钉治疗骨盆后环骨折，节省手术时间，创伤小、出血少，操作简便，只要严格掌握适应证，熟悉局部解剖，认真细致操作，就能达到骨盆后环的稳定，是治疗C型骨盆骨折的可靠方法之一。

Objective： To evaluate clinical effects and safety of fixation for posterior pelvis ring injury with C-arm fluoroscopic guided percutaneous iliosacral screw on supine position. Methods： 21 cases of AO C-type pelvic fracture were treated with percuteaneous iliosacral screws for posterior pelvic ring and external fixation for anterior pelvic ring.then evaluted its clinical effects. Results： After operations, all patients were followed up from 6 to 24 months with an average of 11 months, according to the Matta grading system, the results were excellent in 16 cases, good in 3 cases; the excellent and good rate was 95.23% All fractures were healed well and all patients had no serious pain and recovered well.Conclusions： with percutaneous iliosacral screw fixation for posterior pelvis ring injury on supine position has the advantages including small wound, simple operation, short operation time, less bleeding.It is a reliable method for AO C-type pelvic fracture, only if mastered pelvis constuction and operation carefully.
