摘要 目的探讨卢戈氏液染色对早期食管癌的诊断价值。方法回顾性分析内镜下卢戈氏液染色的3924例中检出经术后病理证实为早期食管癌的36例患者(0.92%)的临床资料。结果 36例早期食管癌患者中,男性26例(72.22%),女性10例(27.78%),男女比例为2.6︰1。50~60岁24例(66.67%),食管中下段35例(97.22%)。充血型及病变面积<2 cm的糜烂型食管癌中,黏膜内癌和黏膜下层癌分别为80.00%和20.00%;病变面积>2 cm的糜烂型食管癌中,黏膜内癌和黏膜下层癌分别为30.76%和69.23%;斑块型食管癌中,黏膜内癌和黏膜下癌分别为42.86%和57.14%;乳头型食管癌中,黏膜内癌和黏膜下层癌分别为100.00%和0%。结论内镜下的卢戈氏液染色可明显提高早期食管癌检出率。该方法操作简便、安全,尤其适合在基层医院推广应用。 【Objective】 To explore the value of Lugol's staining in diagnosis of early esophageal cancer.【Methods】 The clinical data of 3 924 people with Lugol's staining under endoscope were retrospectively analyzed,the detection was confirmed as early esophageal cancer by pathology after operation in 36 cases(0.92%).【Results】 Among the 36 cases of early esophageal cancer,26 were males(72.22%) and 10 were females(27.78%),the male to female ratio was 2.6︰1.The age of 24 patients(66.67%) was between 50 and 60 years.The cancer appeared in lower esophagus in 35 cases(97.22%).In congestive type and erosive type with lesion area of less than 2 cm,intramucosal cancer and submucosal cancer accounted for 80.00% and 20.00%.In the erosive type with lesions larger than 2 cm,the intramucosal cancer and submucosal cancer accounted for 30.76% and 69.23%.In the plaque type,the intra-mucosal cancer and submucosal cancer occupied 42.86% and 57.14%.In papillary carcinoma,the intra-mucosal and submucosal carcinoma occupied 100.00% and 0%.【Conclusions】 Lugol's staining in endoscope can significantly increase liquid dye detection rate for early esophageal cancer.In tip-prone areas,people whether or not having the symptoms of upper digestive tract should have endoscopic examination and conventional staining to prevent misdiagnosis.Endoscopic classification of clinical stage has certain significance,and the method is simple,safe,and particularly suitable for application in the primary hospital.
