血管回声跟踪技术对脑梗死合并糖尿病患者早期颈动脉硬化的研究 被引量:1

摘要 目的:应用血管回声跟踪技术评价脑梗死合并非胰岛素依赖型糖尿病(non-insulin dependent diabetes,NIDD)患者颈动脉弹性的变化。方法:应用血管回声跟踪技术(echo-tracking,ET)检测45例脑梗死合并NIDD患者(cerebral infarction with NIDD,CN组)颈动脉弹性功能改变,并与40例脑梗死患者(cerebral infarction,CR组)及43例正常人(health control,HC组)相比较。收集双侧颈总动脉的二维超声图像,测量双侧颈总动脉内中膜厚度平均值(inti ma-media thickness,I MT)、阻力指数(resistent index,RI)、搏动指数(pulsatilityindex,PI)及收缩期、舒张期比值(systolic phase/diastolic phase,S/D)。应用ET技术实时跟踪描记颈总动脉管壁的运动轨迹,系统自动得出颈总动脉收缩末期内径(diameter of systolic,Ds)、舒张末期内径(diameter of diastolic,Dd)、硬度指数(arterystiffness,β)、压力应变弹性系数(elasticity modulus,Eρ)、顺应性(artery compliance,AC)、增大指数(augmentation index,AI)和脉搏波传导速度(pulse wave velocity,PWVβ)。结果:CN组、CR组的颈总动脉I MT、RI、PI、S/D、β、Eρ、AI及PWVβ均较HC组升高,AC较HC组降低,差异均有统计学意义(P<0.05);CN组颈总动脉I MT、RI、PI、S/D、Eρ、AC及AI测值与CR组比较无显著差异(P>0.05),β与PWVβ较CR组升高(P<0.05),较CR组降低,但比较无统计学意义(P>0.05)。结论:ET技术可敏感地反映出脑梗死合并NIDD糖尿病患者颈动脉弹性的降低。 Objective:To investigate the changes of the elasticity of carotid artery of cerebral infarction with non-insulin dependent diabetes (NIDD) patients with echo-tracking(ET) technique.Methods:A total of 45 patients with cerebral infarction with NIDD(CN group),40 patients with cerebral infarction in the carotid artery but without NIDD(CR group) and 43 healthy subjects as health control group(HC group) were studied with ET technique.The longitudinal view of common carotid artery(CCA) was displayed by two-dimensional ultrasonic imaging and intima-media thickness(IMT) of CCA,resistent index(RI),pulsatility index(PI) and systolic phase/ diastolic phase(S/D) were measured.The elasticity modulus(Eρ),artery compliance(AC),artery stiffness(β),one-point pulse wave velocity(PWVβ) and augmentation index(AI) were individually measured.Results:There were significant differences among CR,CN and HC group.IMT,RI,PI,S/D,β,Eρ,AI and PWVβ of the CN and CR group were higher,but the AC was lower than that of HC group(P0.05).IMT,RI,PI,S/D,β,Eρ,AI and AC have no difference between CN group and CR group(P0.05),although β and PWVβwas of CN group were higher than CR group(P0.05).Conclusion:The elasticity degrading of the carotid artery can be detected sensitively with ET technique.
